冠状动脉旁路移植术56例报道

目的总结分析56例冠状动脉旁路移植术(CABG)的临床经验.方法 1999年10月～2003年11月,我院对56例冠心病患者行冠状动脉旁路移植术.左主干病变4例,单支病变3例,双支病变13例,三支病变36例.全部胸骨正中切口,42例在体外循环下进行,17例在非体外循环下进行.乳内动脉搭桥26根,大隐静脉搭桥128根,人均搭桥2.8支.结果全组死亡1例,其余患者痊愈出院,术后心绞痛完全消失或明显减轻.随访2～48个月效果良好.结论 CABG是治疗冠心病的一种安全有效的方法,合理选择病人,妥善围手术期管理,术中良好的心肌保护及不断提高外科操作技术是提高手术成功率的关键.

Clinical analysis of coronary artery bypass grafting in 56 cases

Objective To review the experience of coronary artery bypass grafting in 56 patients with coronary atherosclerotic heart disease.Methods Between October 1999 and December 2003,56 cases,52 men and 4 women,underwent coronary artery bypass grafting in our institute.Including left main lesion in 4,single stenosis in 3,double stenosis in 13 and triple vessel disease in 36.Median sternotamy was used in all cases,42 cases performed with extracorporeal circulation and 17 cases with off pump coronary bypass grafting(OPCAB).The numbers of internal mammary arterial grafts and saphenous vein grafts was 26 and 128.The mean number of grafts were 2.8 . Results There was one postoperative death.The others recovered smoothly,angina pectoris were totally relieved or alleviated observably.Follow up varied from 2 to 48 months,results are rather satisfactory. Conclusion Coronary artery bypass grafting is a safe and effective method for patients with coronary atherosclerotic heart disease.The key of successful operation are the choice of the patients,the proper management during perioperative peried,satisfactory myocardial protection and accurately anastomsis.
