不同部位脑梗死患者颈部动脉血流动力学改变及临床意义

摘    要:目的:探讨不同部位脑梗死患者急性期颈部动脉血流动力学改变及临床意义。方法:研究对象包括39例左前循环脑梗死患者、76例右前循环脑梗死患者、34例后循环脑梗死患者及59例非脑梗死患者。应用彩色多普勒超声在入院后1周内检测颈部动脉收缩期峰值流速、舒张末期流速、加速度时间、阻力指数。应用t检验或非参数检验对数据进行分析处理。结果:不同部位脑梗死患者分别与非脑梗死患者比较,双侧颈总动脉阻力指数均升高,差异有统计学意义(P<0.05);左、右前循环脑梗死患者分别与非脑梗死患者比较,双侧颈总动脉舒张末期流速均下降,差异有统计学意义(P<0.05)。结论:脑梗死患者普遍存在着双侧颈总动脉高阻力现象,这种现象与脑梗死部位、受累血管无关;脑梗死急性期保持较高平均动脉压对防止病情进展具有重要意义。

Abstract:Aim: To study hemodynamic changes of cervical arteries in the acute stage of different sites cerebral infarction and its clinical significance. Methods:Subjects included 39 left anterior cerebral circulation infarction patients, 76 right anterior cerebral circulation infarction patients, 34 posterior cerebral circulation infarction patients and 59 non-cerebral infarction patients. Color Doppler ultrasonography was employed to measure SPV, EDV, AT, and RI of various cervical arteries within the 1st week after admission. Data were analyzed by t-test or nonparametric tests. Results: Cerebral infarction patients had significantly higher left and right CCA RI when compared with non-cerebral infarction patients (P < 0.05). Left or right anterior cerebral circulation infarction patients had significantly lower CCA EDV when compared with non-cerebral infarction patients (P < 0.05). Conclusion:High resistant hemodynamic disturbances in both left and right CCA were commonly observed in cerebral infarction patients. This phenomenon was not related to the infarction sites and affected blood vessels. It is of significant importance to maintain relatively high average arterial blood pressure in the acute stage of cerebral infarction so as to prevent deterioration of the illness.
