双氯芬酸钠利多卡因联合屈他维林治疗痉挛性腹痛的疗效评价

目的观察和评价双氯芬酸钠利多卡因联合屈他维林治疗痉挛性腹痛的临床疗效和安全性。方法将270例患者随机分成治疗组（144例）和对照组（126例），治疗组采用肌注注射用双氯芬酸钠利多卡因75mg（溶于2ml灭菌注射用水）和肌注注射用屈他维林针剂40-60mg；对照组用哌替啶100mg和阿托品0.5mg联合肌肉注射，观察患者的呼吸、脉搏、心率、血压和疼痛缓解情况，进行疗效及安全性评价。结果治疗组患者疼痛缓解有效率（肾绞痛组：92.86％，胆绞痛组：93.75％）与对照组（肾绞痛组：75.61％，胆绞痛组：54.55％）相比存在统计学差异（P〈0.01），而不良反应发生率无统计学差异（治疗组：1.39％，对照组：3.17％，P〉0.05）。结论双氯芬酸钠利多卡因联合屈他维林治疗痉挛性腹痛使用方便、安全，疗效可靠，且副作用少，值得在急诊中推广使用。

Effect on spastic stomachache with Diclofenac sodium/Dolicaine associated with Drotaverine Hydrochloride

Objective To evaluate the efficacy and safety of Diclofenac sodium/Dolicaine associated Drotaverine Hydrochloride in treating spastic stomachache. Methods 270 patients were divided into two groups randomly： the treatment group and the control group. 144 patients in the treatment group with the Diclofenac sodium/Dolicaine 75mg, associated with Drotaverine Hydrochloride 40 -60 mg, and 126 patients in the control group with Pethidine 100 mg by intramuscular injection and Atropine 0. 5 mg by intramuscular injection R, P, HR, BP of these patients were observed to analyse their efficacy and safety. Results The effective rate of the treatment group was 92.86% in nephric colic （82） and 93.75 % in biliary colic （44） ,only 2 patients accompanied transiant dizziness, that of the control was 75.61% and 54. 55%. There were 4 patients accompanied dry mouth, palmus, blurred eyes and urine retention in different degree. The two groups had significant difference（P〈0.01）. The side effect of the treatment group was less than that of the the control group. Conclusion Diclofenac sodium/Dolicaine associated Drotaverine Hydrochloride is effective medicine to spastic stomache with convenience, safety and reliable curative effect.
