肾上腺髓性脂肪瘤的影像学诊断

目的 探讨肾上腺髓性脂肪瘤影像学诊断及鉴别诊断价值。方法 搜集肾上腺髓性脂肪瘤10例，全部病例行彩色多普勒超声检查，CT扫描7例，分析其超声及CT表现。结果 彩色多普勒超声检查肿块呈高回声8例；混合回声2例，内部伴不规则低回声；5例CT表现为境界清晰、圆形或卵圆形低密度肿块，CT值小于-20Hu，内见条索状或团片状小阴影。2例呈混杂密度，以脂肪密度为主，其中1例肿瘤破裂并腹腔出血。结论 彩色多普勒超声及CT检查对肾上腺髓性脂肪瘤诊断及鉴别诊断具有重要价值，可为临床治疗方案选择提供必要的影像学依据。

The imaging diagnosis of adrenal myelolipoma

Purpose To investigate the diagnosis and differentiation of adrenal myelolipoma (AML) by ultrasound and CT. Methods 10 cases of AML were collected. All cases underwent Color Doppler Ultrasound and 7 cases underwent CT plain scanning. The article intended to analyze the CT and ultrasonographic appearances of AML. Results The Doppler sonographic appearance of AML was displayed as hyper - echo mass in 8 cases , hetero - echo mass in 2 cases with irregular low or cystic echo regions. CT demonstrated well - circumscribed, round or oval low density mass with small stripe and patchy shadows in 5 cases, CT value below - 20Hu. 2 cases were hetero-masses giving priority to fat density and 1 case ruptured with abdominal cavity hemorrhage. Conclusion Doppler ultrasonography and CT have important value in the diagnosis and differentiation of AML, able to provide necessary imaging evidence for the choice of clinical treatment.
