摘    要:目的探讨两种肾动脉结扎法建立肾性高血压伴肾衰大鼠模型及其生化指示的评价。方法采用外科切除右肾并结扎左肾动脉上段分支及中、下段分支,继续饲养8周,实验前尾袖法测定大鼠的血压,断头取血,放免法测定血清血管紧张素II(AII)和内皮素(ET)。双缩脲法测定24 h尿蛋白,生化指标采用全自动生化分析仪检测。结果(1)与正常对照组相比,两种手术组血压都明显升高(126.67±8.5 VS 177.34±9.87,174.83±13.38,P<0.05),而手术组之间无明显差异(177.34±9.87,174.83±13.382,P>0.05)。两手术组血清中AII、ET水平明显增高(AII:2355.43±388.79,2556.28±408.33 VS 1546.78±312.96,P<0.05;ET:294.24±36.56,311.56±39.46 VS225.93±63.57,P<0.05)。尿24 h总蛋白量也明显高于正常组(25.76±12.45,23.81±5.94 VS 2.84±1.63P<0.05)(3)与正常对照组比,三组之间血清ALT,ALB及Trig无明显差别,但手术组AST、GLU、CHOL却有明显升高。(4)BUN和CREA与假手术组相比都有明显升高(BUN:13.39±2.18 12.87±1.61 VS 4.31±0.59P<0.05;CREA:59.85±9.19 55.47±5.80 VS 20.00±5.78P<0.05)。结论(1)采用两种手术法构建肾性高血压伴衰功能不全模型血清生化指标无明显差异;(2)大鼠背外侧手术入路行左肾动脉前支结扎建立肾性高血压伴肾衰模型手术操作更为简便;(3)肾动脉狭窄可引起血清AII、ET水平增高,其可能形成肾性高血压的并加速肾衰的重要原因。

Abstract:Objective To investigate renal hypertension associated with renal failure by ligating different branches of the left renal artery in rats and to assess the related serum biochemical indicators.Methods Rats underwent ligation of the posterior branch(n=10) or the anterior and middle branches(n=10) of the left renal artery followed by right nephrectomy.Another 10 rats served as controls.Eight weeks after the operation,tail cuff blood pressure(BP) was measured and the rats were sacrificed using decapitation with the serum samples obtained for the measurement of serum angiotensin II(AII) and endothelin(ET) by radioimmunity.Results BP,serum AII,ET,AST,GLU,CHOL,BUN and CREA levels,as well as quantity of 24 hours' urinary protein of rats in the two operation groups were significantly higher than those in the(control group,while no significant difference was observed between the two operation groups.As for serum ALT,ALB and Trig,no significant difference was observed between the operation groups and the control group.Conclusions(1)A rat model of renal hypertension associated with renal failure can be successfully established using the two operative methods with no difference in related to serum biochemical indicators;(2)It is more convenient to establish a rat model of renal hypertension associated with renal failure by ligating the anterior branch of the left renal artery with a dorsolateral approach;(3)Renal arterial stenosis may cause an increase in serum AII and ET levels,which may play an important role in causing renal hypertension and accelerating renal failure.
