肺功能检查对慢性阻塞性肺疾病患者预后的评估

目的探讨肺功能检查对慢性阻塞性肺疾病(COPD)患者预后评估的临床分析。方法对2008年7月至2011年8月北京王府中西医结合医院所收治的53例COPD患者经肺功能检测的临床资料进行回顾性分析。结果根据FEV1%和EV1/FVC值对53例COPD患者的病情程度评估为:中度(Ⅱ级)26例(49.06%),重度(Ⅲ级)23例(43.40%),极重度(Ⅳ级)4例(7.5%)。经支气管扩张试验显示,5例患者为支气管扩张试验呈阳性,48例患者为支气管扩张试验呈阴性。通过对比COPD患者肺功能情况,支气管扩张试验阳性患者的用力肺活量(FVC)、肺活量(VC)、呼气容积(FEV1)、吸入一氧化碳弥散量(DLCO)均显著优于试验阴性的患者(P<0.05)。结论应全面推广应用肺功能测定技术,尤其对患肺疾病的危险人群定期进行肺功能检查,将有利于及早了解疾病进程,对COPD患者的预后评估有重要意义。

Lung Function Tests of COPD Patients for Predicting the Prognosis

Objective To investigate the clinical analysis of the lung function tests in chronic obstructive pulmonary disease(COPD)in predicting the prognosis.Methods The analysis on clinical data of 53 COPD cases undergoing pulmonary function testing from 2008 July to 2011 August in Beijing Royal Integrative Medicine Hospital was done.Results The 53 diagnosed cases according to their FEV1% and FEV1/FVC value,were graded into:moderate(grade Ⅱ):26 cases(49.06%);severe(gradeⅢ):23 cases(43.40%);extremely severe(Ⅳ):4 cases(7.5%).Bronchiectasis test of 5 cases was positive,and the remaining 48 was negative.By comparing the pulmonary function of COPD patients,forced vital capacity(FVC),vital capacity(VC),expiratory volume(FEV1),and diffusing capacity of CO(DLCO)of the patients with positive result were significantly better than the negative(P<0.05).Conclusion The application of pulmonary function testing should be promoted,especially for those populations at risk;the regular pulmonary function test will help in early understanding of COPD progress and the prognosis.
