几个生化指标对鉴别不同病因急性胰腺炎的意义

目的：我们进行了一次回顾性研究来确定几个生化指标在鉴别酒精性和非酒精性胰腺炎中的用处．方法：把140个患者分成A组（酒精性胰腺炎）和NA组（非酒精性胰腺炎）．NA组又进一步分为B组（胆源性胰腺炎）和NANB组（非酒精、非胆源性胰腺炎）．测定血、尿淀粉酶、血脂肪酶、脂肪酶／淀粉酶、AST、ALT、ALP、胆红素、r－GT、和平均红细胞容积．结果：酒精性胰腺炎患者血淀粉酶，ALT、AST、ALP和尿淀粉酶显著降低（P＜0．01）．酒精性胰腺炎患者的平均红细胞容积、L／A显著增高．脂肪酸、胆红素、r－GT在酒精性和非酒精性胰腺炎间无显著差异．结论：生化指标和其它临床特点一起可以帮助我们早期鉴别酒精性和非酒精性急性胰腺炎．

The clinical significance of several biochemical parameteres in various causes of acute pancreatitis

Aims:We undertook a retrospective study to specify the uses of various biochemical indexes in identifying alcoholic and nonalcoholic pancreatitis.Methods: One hundred forty patients were divided into group A (patients with alcoholic pancreatitis) and NA (patients with nonalcoholic pancreatitis). Group NA was further subdivided into group B (biliary pancreatitis) and NANB (nonalcoholic, nonbiliary pancreatitis). The values of blood amylase and amylase in urine, nemolipase AST ALT ALP r-glutamyl transferase, bilirubin. lipase/amylase.and mean corpuscular volume were determined. Results: blood amylase. ALT AST ALP(P<0.001 ), and amylase in urine (P<0.01 ) were significantly lower in patients with alcohotic pancreatitis. mean corpuscular volume and lipase/amylase were significantly higher in patients with alcoholic pancreatitis (P<0.001).There were no differences in lipase. bilirubin, and r-glutamyl transtferase between patients with alcoholic pancreatitis and those with nonalcoholic pancreatitis. Conclusion : Various biochemical indexes together with other clinical symptoms can help identify alcoholil and nonalcoholic acute pancreatitis.
