利咽解毒方防治大鼠急性放射性口腔黏膜炎作用机制的研究

摘    要:目的观察自拟利咽解毒方对大鼠急性放射性口腔黏膜炎(ROM)的抑制作用及作用机制。方法建立大鼠ROM模型,将50只大鼠随机分为5组,即空白组、模型组、西药组、利咽解毒方组高、低剂量组,每组10只。除空白组不予特殊处理外,其余4组实验组大鼠均予等剂量、等照射野和等源距的ν线照射,观察各组大鼠的口腔颊黏膜炎出现及发展情况,放疗1周后切取口腔颊部黏膜,比较各组标本中的缺氧诱导因子-1α(HIF-1α)、环氧化酶-2(COX-2)、白介素-6(IL-6)的表达。结果自拟利咽解毒方组、西药组大鼠HIF-1α、COX-2、IL-6表达水平明显低于模型组,高于空白组(P<0.05);低剂量及高剂量组与西药组相比,两组差异有统计学意义(P<0.05),其中低剂量利咽解毒方组与高剂量组相比,两组差异有统计学意义(P<0.05)。结论利咽解毒方可能通过下调颊黏膜组织中HIF—1α、COX-2、IL-6水平,抑制放疗引起的ROM。

Abstract:Objective: To discuss the inhibition effects and the mechanism of Liyanjiedu Decoction on rats with radiation-induced oral mucositis. Methods: Acute radiation-induced oral mucositis model was established and 50 model rats were randomly divided into 5 groups as follows: blank group,model group,western medicine group, Liyanjiedu Decoction low-dose group (low dose group for short), and Liyanjiedu Decoction high -dose group (high dose group for short) . Rats in each group except blank group were given the same dose equivalent radiation field and equally spaced gamma irradiation. Then the appearance and development of oral mucositis in each group were observed. Expressions of HIF-1α, COX-2 and IL-6 in the sample were detected by immunohistochemistry a week after radiotherapy. Results: The level of HIF-1α,COX-2 and IL-6 in Liyanjiedu Decoction low-dose group ,highdoes group and western medicine group were significantly lower than model group, but higher than blank group. The difference was statistically significant(P〈0.05). The differences among low-dose group,high-does group,and western medicine group were statistically significant (P〈0.05). Conclusion: Liyanjiedu Decoction may lower expressions of HIF-1α,COX-2 and IL-6 in buccal mucosa tissues to inhibit acute radiation-induced oral mucositis caused by radiotherapy. It explores pathogenesis of acute ROM from molecular level,prove the curative effect of TCM treating acute ROM from angle of experiment, and provide experimental basis for clinical research.
