射频消融术治疗良性子宫出血性疾病的临床应用

目的 探讨射频消融术治疗良性子宫出血性疾病的临床特点和疗效.方法 将2008年5月-2011年9月我院行射频消融术治疗的良性子宫出血性疾病62例（A组）与同期因上述疾病行宫腔镜手术治疗51例（B组）以及子宫切除手术治疗43例（C组）进行比较分析.结果 射频消融术（A组）治疗异常子宫出血症状的总有效率为95.2％,其中功血的有效率为94.7％,粘膜下肌瘤为100％,腺肌症为87.5％,宫内膜息肉为100％;B组的总有效率为96.1％;C组的治愈率为100％.虽然C组的疗效优于A、B两组,但其手术时间、术中出血量、保留尿管及住院时间均明显多于A、B组,差异有统计学意义（均P＜0.05）.结论 射频消融术治疗良性子宫出血性疾病操作简单、安全、有效、微创,易被广大患者接受,有望同宫腔镜手术一样成为替代子宫切除术治疗良性子宫出血性疾病的方法之一,值得临床推广应用.

Clinical application of radiofrequency ablation on benign uterine bleeding diseases

Objective To investigate clinical characteristics and therapeutic effects of radiofrequency ablation on uterine benign bleeding diseases.Methods 62 patients with uterine hemorrhage （group A） treated with radiofrequency ablation,51 patients with uterine hemorrhage （group B） treated with uteroscope and 43 patients treated with hysterectomy were analyzed.Results The total effective rate in group A was 95.2％.The curative effect rate was 94.7 ％ in dysfunctional uterine bleeding,100 ％ in submucosal leiomyoma,87.5 ％ in uterine adenomyoma,and 100％ in endometrial polyp.The total effective rate in group B was 96.1％.Group C cure rate was 100％.Although the efficacy of group C was better than that of A,B groups,but its operation time,blood loss,urinary retention and length of stay were significantly greater than that of A,B groups （P＜0.05）.Conclusion The treatment method to uterine benign bleeding disease by radio frequency ablation is simple,safe and effective.It is easy to be accepted by the patients and deserves further clinical applications.
