摘    要:【目的】探讨癌胚抗原(CEA)、细胞角蛋白19片段(CYFRA21—1)、神经特异性烯醇化酶(NSE)联合检测对肺癌及其各病理类型的诊断价值。【方法】采用电化学发光法对148例肺癌患者,83例健康对照者,73例肺部良性疾病组,进行CEA、CYFRA21-1、NSE检测,比较单项肿瘤标志物及三项联合检测的阳性率和特异性。【结果】CEA、CYFRA21—1、NSE分别在腺癌、鳞癌、小细胞癌中表达水平最高,其阳性率分别为75.9%、90.3%、85.7%,与其他单项肿瘤标志物比较差异有显著性。三项联合检测阳性率在肺癌及各病理类型中,与单项肿瘤标志物比较差异有显著性。【结论】血清CEA、CYFRA211、NSE三项联合检测对诊断肺癌有重要意义,同时对肺癌的病理类型的判断具有实用诊断价值。

Abstract:Objective]To investigate the value of combination measurement of carcinoembryonic antigen(CEA), CYFRA21-1 and neuron-specific enolase(NSE) for the diagnosis of lung cancer with various pathological types. Methods] A total of 148 cases of lung cancer, 83 healthy controls and 73 benign lung disease were enrolled in this study. CEA, CYFRA21 1 and NSE were measured by electrochemiluminescence method. The positive rate and speci ficity were compared between simple tumor marker and three tumor markers. Results] The expressions of CEA, CYFRA21-1 and NSE in adenocarcinoma, squamous cell carcinoma and small cell carcinoma were the highest, and their positive rates were 75.9%, 90.3% and 85.7%, respectively. There was significant difference between these three tumor markers and other simple tumor marker. There was also significant difference in the positive rate in lung cancer with various pathological types between the combination of these three tumor markers and one of these markers. Conclusion] Combination measurement of serum CEA, CYFRA21 1 and NSE has important significance for diagnosing lung cancer, and also has useful value for distinguishing the pathological types of lung cancer.
