脑卒中与高血压病分级及危险度分层关系与降压终点的选择

目的：评价新的高血压分级标准预测脑卒中的价值，探讨抗高血压药物的理想降压水平。方法：以脑卒中做为高血压病的终点事件，对可进行临床评价的致死性脑卒中与非致死性脑卒中287例患者，按照新的高血压诊断标准中定量预后分级与危险度分层的相关资料进行统计分析，探讨后者预测脑卒中的临床价值；根据脑卒中患者发病前的平均血压控制水年并结合非高血压性脑梗死患者的对照分析，探讨抗高血压药物降压治疗的理想终点水平。结果：287例脑卒中患者中257例（89．5％）为3级高血压，273例（95．1％）为极高危险组；可评价发病前血压水平的198例高血压脑卒中患者中，12例（6．06％）低于HOT水平，而同期非高血压性脑梗死发病21树，提示较低的血压水平至少没有增加脑梗死的发病率。结论：新的高血压分类标准对预测脑卒中事件有重要临床价值，抗高血压药物的理想降压终点应参照HOT标准。

The relationship between stroke and grade, dangerous degree of hypertension, and hypertension optimal treatment

To confirm the relation between stroke and new standard of hypertension classification and hypertension optimal treat ment. MethodsTaking stroke to be trminal even of hypertesion and according to the new standard of hypertension chassification the clinical data of 287 patients with stroke were analysed. Studying assessabal average controlled blood pressure level of 198 patients with stroke and Controlling 21 cases of cerebral infarction hadnt hypertension to confirm hypertension optimal Results: 257 of 287 - of stheke (89.5% ) had hypertension Grade 3, 273 of 287 cases (95.1%) had highest dangerous degree, 12 of 189 assessable cases of stroke(6.06% ) had lowering blood pressure level under HOT, 7 of this 12 cases was hyperteive cerebral infarction 21 patients suffered from no hypertensive cerebral infarection at same time . This pointal out that low blood pressure level didn't increase danger of cerebral infarction at leas. Conclusion: hpertension Grade and dangerous degree of new hypertension classification may calculate stroke event of patients with hypertesion , HOT may make a treation guide of hypertension at a stroke angle
