单侧、双侧经皮椎体后凸成形术治疗骨质疏松性椎体压缩骨折Meta分析

摘    要:目的 系统评价单侧、双侧经皮椎体后凸成形术治疗骨质疏松性椎体骨折(OVCF)的疗效.方法 检索PubMed、Embase、万方数据库、知网数据库、维普数据库,手工检索相关文献的参考文献及主要中英文骨科杂志,收集单侧、双侧经皮椎体后凸成形术治疗骨质疏松性椎体骨折的随机对照试验,纳入文献用改良的Jadad量表评价文献质量后采用Stata 15软件进行Meta分析.结果 本研究纳入7篇文献.单侧、双侧经皮椎体后凸成形术治疗的手术时间加权均数差(WMD)=-20.37,95%可信区间-24.26~-16.48]、骨水泥用量(WMD=-1.93,95%可信区间-2.51~-1.35)比较,差异有统计学意义(P<0.01).术后早期视觉模拟疼痛评分、术后早期Cobb角变化、骨水泥渗漏率比较,差异无统计学意义(P>0.05).结论 与双侧经皮椎体后凸成形术比较,单侧经皮椎体后凸成形术能有效减少手术操作时间,减少骨水泥用量,在Cobb角变化、骨水泥渗漏率方面无明显差异.

Unilateral and bilateral percutaneous kyphoplasty for the treatment of osteoporotic vertebral compression fractures:a Meta-analysis study

Abstract:Objective To systematically evaluate the efficacy of unilateral and bilateral percutaneous kyphoplasty in the treatment of osteoporotic vertebral compression fracture(OVCF).Methods Searching PubMed,Embase,Wanfang database,HowNet database and VIP database and manually searched related references and major Chinese and English orthopedic journals,one-sided and two-sided percutaneous kyphoplasty was used to treat osteoporotic vertebral fractures,and a Meta-analysis was performed with Stata 15 software after a modified Jadad scale was used to evaluate the quality of the literature.Results Seven literatures were included in this study.There were statistically significant differences in operative timeweighted mean difference(WMD)=-20.37,95%CI:-24.26 to-16.48,P<0.01]and amount of bone cement(WMD=-1.93,95%CI:-2.51 to-1.35,P<0.01)between unilateral and bilateral percutaneous KYP.There was no statistical significance in early postoperative visual analogue score,early postoperative Cobb Angle change and bone cement leakage rate(P>0.05).Conclusion Compared with bilateral percutaneous kyphoplasty,unilateral percutaneous kyphoplasty can effectively reduce the operation time and the amount of bone cement.There is no statistical significance in Cobb Angle change and bone cement leakage rate.
