无创正压通气治疗COPD急性加重期合并2型呼吸衰竭患者的临床效果

目的分析无创正压通气治疗慢性阻塞性肺疾病(COPD)急性加重期合并2型呼吸衰竭患者的临床效果。方法选取本院2018年1月至2019年12月收治的50例COPD急性加重期合并2型呼吸衰竭患者为研究对象,根据治疗方式的不同将其分为对照组(常规治疗)和研究组(常规治疗基础上开展无创正压通气治疗),各25例。比较两组治疗前、后血气分析指标(pH、PaO2、PaCO2、SpO2)、HR、RR、住院时间、6MWT、FVC、FEV1。结果治疗后,研究组的pH、PaO2、SpO2显著高于对照组,PaCO2显著低于对照组,差异具有统计学意义(P<0.05)。治疗后,研究组的HR、RR显著低于对照组,差异具有统计学意义(P<0.05)。研究组的住院时间短于对照组,出院时6MWT、FVC、FEV1优于对照组(P<0.05)。结论无创正压通气治疗COPD急性加重期合并2型呼吸衰竭对改善患者血气分析指标、HR、RR、6MWT有积极意义,值得临床推广应用。

Clinical effect of noninvasive positive pressure ventilation in patients with acute exacerbation of COPD complicated with type 2 respiratory failure

Objective To analyze the clinical effect of noninvasive positive pressure ventilation in patients with acute exacerbation of chronic obstructive pulmonary disease(COPD)complicated with type 2 respiratory failure.Methods A total of 50 patients with acute exacerbation of COPD complicated with type 2 respiratory failure treated in our hospital from January 2018 to December 2019 were selected as the research objects.According to the different treatment methods,the patients were divided into control group(routine treatment)and study group(noninvasive positive pressure ventilation on the basis of routine treatment),with 25 cases in each group.Blood gas analysis indexes(pH,PaO2,PaCO2,SpO2),HR,RR before and after treatment,hospital stay,6 MWT,FVC and FEV1 were compared between the two groups.Results After treatment,the pH,PaO2 and SpO2 of the study group were significantly higher than those of the control group,and PaCO2 was significantly lower than that of the control group,the differences were statistically significant(P<0.05).After treatment,the HR and RR of the study group were significantly lower than those of the control group,and the differences were statistically significant(P<0.05).The hospital stay in the study group was shorter than that of the control group,and 6 MWT,FVC,FEV1 at discharge were better than those of the control group(P<0.05).Conclusion Noninvasive positive pressure ventilation in the treatment of acute exacerbation of COPD complicated with type 2 respiratory failure has positive significance in improving blood gas analysis indexes,HR,RR and 6 MWT results,which is worthy of clinical promotion and application.
