布氏杆菌性脊柱炎的CT与MRI表现

目的探讨布氏杆菌性脊柱炎(BS)的CT、MRI表现,以提高该病的诊断水平。方法回顾性分析18例BS患者的CT、MRI图像特征。结果18例患者均行CT及MRI检查,共48个椎体受累,腰椎最多,胸椎次之,颈椎最少。CT显示42个病椎,24个椎体仅表现为骨质增生、硬化,18个椎体有骨质破坏并不同程度骨质增生、硬化,2例椎间隙变窄,2例椎旁脓肿。MRI显示48个病椎,6个椎体表现为骨髓水肿,15个椎体骨质破坏伴不同程度的骨髓水肿及骨质增生硬化,27个椎体骨髓水肿伴椎体骨质增生、硬化,4例椎间盘T2WI信号增高,2例椎间隙变窄,4例椎旁脓肿,1例硬膜外脓肿。结论BS的CT、MRI表现具有一定特征性,结合实验室检查及临床病史有助于明确诊断。

CT and MRI features of brucellosis spondylitis

Objective To investigate the CT and MRI features of brucellosis spondylitis(BS)in order to improve the diagnostic level of the disease.Methods The CT and MRI features of 18 patients with BS were analyzed retrospectively.Results All 18 patients underwent CT and MRI examinations.A total of 48 vertebral bodies were involved,the lumbar spine was the most involved,the thoracic spine was the second,and the cervical spine was the least.CT showed 42 diseased vertebral bodies,24 vertebral bodies showed only bone hyperplasia and sclerosis,18 vertebral bodies had bone destruction with varying degrees of bone hyperplasia and sclerosis,and there were 2 cases of intervertebral space narrowing and 2 cases of paravertebral abscess.MRI showed 48 diseased vertebral bodies,6 vertebral bodies had showed bone marrow edema,15 vertebral bodies had bone destruction with varying degrees of bone marrow edema and bone hyperplasia and sclerosis,27 vertebral bodies had bone marrow edema with vertebral bodies bone hyperplasia and sclerosis;the T2WI signal of 4 cases of intervertebral disc increased,and there were 2 cases of intervertebral space narrowing,4 cases of paravertebral abscess and 1 case of epidural abscess.Conclusion The CT and MRI features of BS have certain characteristics,which combined with laboratory examination and clinical history can help to make a definite diagnosis.
