微骨窗入路与常规开颅治疗高血压脑出血的疗效分析

【目的】探讨微骨窗显微手术与常规开颅手术治疗高血压脑出血的疗效。【方法】112例符合纳入标准的高血压脑出血患者分为微骨窗显微手术组（n=70）和常规开颅手术组（n=42）进行手术治疗。【结果】微骨窗显微手术组患者术后死亡率明显低于常规开颅手术组，差异有显著性（P〈0．05）；术后日常生活活动能力（ADL）亦优于常规开颅手术组（P〈0．05）。【结论】与常规开颅手术相比，微骨窗开颅显微手术具有创伤小、时间短、准确可靠，患者预后好等优点，是高血压脑出血一种理想的手术方法。

Analysis on Curative Effect of Keyhole Approach Microsurgery and Conventional Craniotomy for Treating Patients with Hypertensive Cerebral Hemorrhage

Objective] To explore the curative effect of mierosurgery by keyhole approach and convention- al craniotomy for the patients with hypertensive cerebral hemorrhage（HCH）. Methods] A total of 112 patients with HCH corresponded to the inclusive criteria were divided into microsurgical treatment by keyhole approach group（ n = 70）and conventional treatment by craniotomy group（ n = 42）at random. Results] There was significant difference in postoperative death rate between microsurgical treatment by key hole approach group and conventional craniotomy group （ P 〈0.05）. The activity of daily living （ADL） of patients treated with mierosurgery hy key hole approach was significantly superior to that of patients treated with conventional craniotomy （ P〈0.05）. Conclusion]Compared with conventional craniotomy, the microsurgery by keyhole has advantages of micro-invasion, short time, good prognosis, accuracy and reliability, so it is the ideal surgical choice for patients with HCH.
