胸大肌肌皮瓣在Ⅲ、Ⅳ期舌癌术后缺损修复中的应用

目的 :舌癌的手术常造成舌大部缺损或全舌缺损 ,进而影响舌的功能。为修复Ⅲ、Ⅳ期舌癌的术后缺损 ,我们采用了胸大肌肌皮瓣修复术后缺损。方法 :我们自 1 992～ 1 996年对 1 0例Ⅲ、Ⅳ期舌癌病人行舌部分切除并行同侧颈清扫术后 ,在同侧胸壁下方取适当大小的带蒂岛状胸大肌皮瓣 ,使肌瓣内含胸肩峰动脉及静脉。结果 :胸大肌岛状皮瓣供皮区隐蔽 ,血运好、成活率高 ,皮瓣组织与舌体近似 ,弹性好可随残舌运动 ,同时此肌瓣可提供的可修复面积大 ,所以术中切除彻底 ,避免复发。结论 :胸大肌肌皮瓣在Ⅲ、Ⅳ期舌癌修复中具有极高的使用价值

Application with Skin Flap of Pectoralis Major in Postoperative Defect of Tongue Carcinoma at Stage III and

Objective: To discuss the application with pectoralis major in postoperative defect of tongue carcinoma at stage III and IV. Methods: 10 cases of tongue carcinoma patients at stage III and IV were underwent the operation of partial glossectomy from 1992~1996. The available size of skin flap of pectoralis major with pedicle was transferred with cervical clearance at same side simultaneously. Results: All skin flaps were survived with good blood supply and latent position. The flap tissues were similar with tongue body and had good function. Conclusions: This method could supply large area of skin flap to repair the defect so that the operation could make clearance of tongue lesions thoroughly and avoid recurrence. It could have curative value to repair tongue defect due to glossectomy at stage III and IV of tongue carcinoma.
