亚砷酸经肝动脉化疗栓塞对兔肝VX2移植瘤生长及转移的影响

摘    要:目的观察亚砷酸碘油制剂在兔肝VX2移植瘤化疗栓塞治疗中对肿瘤生长及转移的影响。方法将40只肝内荷瘤兔分成空白对照,单纯碘油栓塞,常规阿霉素碘油栓塞,亚砷酸碘油栓塞及亚砷酸阿霉素联合碘油栓塞共5组。依分组不同进行肝动脉化疗栓塞术并记录肿瘤生长和转移情况。结果本组40例肝脏种植肿瘤2周末时平均体积之间没有明显差异,5周末时各实验组治疗后肿瘤体积都增大,但与对照组相比体积明显减小(P<0.01),内部可见更大片状坏死(P<0.01),尤以亚砷酸用药组明显。较无亚砷酸组相比,亚砷酸用药组可以明显减少远隔转移的数量(P<0.01);其中联合用药组较常规化疗栓塞组更明显减少远隔转移数量(P<0.05)。结论亚砷酸在肝动脉化疗栓塞术中应用可以抑制瘤灶的生长和远隔转移。亚砷酸与常规化疗药物联合应用能进一步增强抗肿瘤转移效应。

Abstract:Objective To study the effects of arsenic trioxide combined with transarterial chemoembolization on the growth and metastasis of the hepatic implanted VX2 tumor in rabbits. Methods After 2 weeks of inoculation of VX2 tumor into the livers, 40 rabbits were randomly divided into 5 groups and treated with different methods. Group A was control group without any treatment, group B as UFLP embolization group, group C as routine TACE group with UFLP and ADM, group D with UFLP and arsenic trioxide and group E combined UFLP with ADM and arsenic trioxide. The implanted hepatic tumors were inspected with enhanced-CT and the size and the necrotic area of tumor were measured. The metastasis in liver and distal organ was recorded. Results Five weeks after inoculation, the mean volume of implanted tumor of five groups from A to E were (70.58±13.08) cm3, (55.33±11.56) cm3, (9.64±3.69) cm3, (3.76±0.27) cm3 and (1.81±0.39) cm3; and the necrosis rate were (14.61±5.30)%, (40.99±13.23)%, (53.76±5.63)%, (57.25±6.68)%, and (48.02±9.57)%, respectively. Compared with control group A, the significant difference was seen in each treatment group (P<0.01). Compared with group B and C, group D and E with arsenic trioxide could inhibit the tumor growth significantly (P<0.01). Group B to E had larger necrosis area than group A (P<0.01), but there was no significant difference between each group (P>0.05). The incidence of metastasis in liver and distal part were 8/8 (100%) and 8/8 (100%) in group A and B, 7/8 (87.5%) and 6/8 (75%) in group C, 5/8(62.5%) and 2/8 (25%) in group D, 4/8 (50%) and 1/8 (12.5%) in group E. The group D, E with arsenic trioxide could prevent tumor metastasis significantly than group A,B,C (P<0.05). Conclusion Chemoembolization combined with arsenic trioxide can not only inhibit the growth of the implanted tumor, but also prevent the tumor from intrahepatic and distal metastasis.
