产超广谱β-内酰胺酶细菌106株耐药性调查

摘    要:目的 了解临床产超广谱 β 内酰胺 (ESBLS)酶细菌的检出率 ,并对其耐药性进行探讨。 方法 菌株鉴定采用法国梅里埃公司API系列产品 ,药敏试验采用纸片扩散法 ,根据产ESBLS 酶细菌调查检测结果 ,结合临床 ,分析其发生及发展情况。 结果  85 8株革兰阴性杆菌中检出产ESBLS 酶菌10 6株 ,检出率 12 35 % ;其中 4 1株来源于医院感染患者的标本 ,其检出率为 34 17% ;6 5株来源于非医院感染患者的标本 ,其检出率为 8 81% ,显著低于医院内感染病人 (P <0 0 1)。产ESBLS 酶菌对氨苄西林的耐药率达 94 34% ,耐药率最低为亚安培南 4 7%。 结论 医院感染病人产ESBLS 酶细菌的发生率明显高于普通住院病人 ,产ESBLS 酶细菌对大多数常用抗生素耐药率超过 5 0 %。

Abstract:Objective To investgate the positive rate of ESBL S producing microorganism and drug resistance, and to compare positive rates of ESBL S producing microorganism between the ordinary patients and the ones with nosocomial infection Methods Bacteria was identified by API series reagents, and drug sensitive test was adopted to K B slip diffuse methods Positive rate was calculated with ESBL S producing microorganism and clinical cases Results There were 106 isolates of ESBL S producing Gram negative bacilli in 858 isolates, and positive rate was 12 35% 41 in 106 isolates of ESBL S producing microorganism rooted in nosocomial infection, and positive rate was 34 17% 65 isolates of ESBL S producing microorganism rooted in sample of common patients, and positive rate was 8 8%, far less than that of nosocomial infection( P <0 01) ESBL S producing microorganism of resistence to ampicillin extended to 94 34%, and that to imipenem was 4 7% Conclusions Positive rate of ESBL S producing microorganism in nosocomial infection is significantly higher than in the ordinary patients Resistance rate of ESBL S producing microorganism to most antibiotics exceeds to 50%
