摘    要:目的观察早产儿不同食物早期微量管饲时促胃液素及胃肠道功能变化。方法将33例早产儿随机分为糖水组、牛奶组,分别管饲5%葡萄糖和早产儿奶,每次喂养2 m l/kg,q3h,每12h增加1 m l/kg,同时予静脉营养。糖水组喂养3 d后逐渐添加早产儿奶。分别于喂养前、喂养后d 1、d 3、d 7测空腹血促胃液素(gastrin,Gas),比较血促胃液素、喂养不耐受发生率及达全肠道喂养时间。结果血Gas:糖水组加奶前各时段差异无显著意义(P>0.05);牛奶组逐日增高,同时牛奶组较糖水组明显增高(P<0.05)。喂养不耐受率:牛奶组较糖水组明显降低(P<0.05)。达全肠道喂养时间:牛奶组明显短于糖水组(P<0.05)。结论早期微量管饲时,奶汁能更好地促进Gas分泌,促进胃肠道成熟。

Abstract:Objective To investigate the changes of serum gastrin and the gastrointestinal function in premature infants who were fed with different foods by tube.Methods Thirty-three premature infants were divided into 2 groups:the glucose group who were fed with 5% glucose and the mild group who were fed with the milk for premature.The volume of feeding were 2 ml/kg every time,q 3 h,and increased 1 ml/kg per 12 h,meanwhile the intravenous feeding were given.The milk were given to the glucose group 3 days later.The serum Gas were measured before feeding and within d_1,d_3,d_7.The serum Gas,the period to full enteral nutrition and the feeding intolerance rate were compared. Results The serum Gas had no significant difference between each periods in glucose group before feeing with milk(P>0.05),and it was higher than that before in milk group,and were obviously higher in milk group than that in glucose group at the same time(P<0.05).The feeding intolerance rate in milk group were less than that in glucose group(P<0.05).The period to full enteral nutrition in milk group were obviously shorter than that in glucose grorp(P<0.05).Conclusions The milk for prematures during earing early minimal tube feeding can increase the serum Gas and develope gastrointestinal function.
