下呼吸道感染死亡病例痰培养及药敏试验分析

目的 了解下呼吸道感染死亡病例的痰菌状况。方法 对72例下呼吸道感染患者痰菌分离，经培养鉴定共取得121株致病菌，药敏试验采用K-B纸片法，结果 G^-菌占72.7%，主要致病菌依次为铜绿假单胞菌24.0%，金黄色葡萄球菌20.7%，产气杆菌19.0%，药敏结果表明细菌耐药明显增强，亚胺培南，环丙沙星，头孢哌酮/舒巴坦为大多数细菌敏感。结论 下呼吸道感染死亡病例的致病菌仍G^-菌为主，产气杆菌检出率高，细菌耐药性明显增强，今后应加强病原菌培养及药敏5监测以指导临床科学使用抗生素。

Sputum Culture and Drug Sensitivity of Pathogens in Deceased Patients with Lower Respiratory Tract Infection

OBJECTIVE To investigate the distribution and drug sensitivity of bacteria from deceased patients with lower respiratory tract infection. METHODS After gargling several times sputum samples from lower respiratory tract were collected from which 121 stains were obtained. Drug sensitivity was tested by Kirby Bauer disk sensitivity method. RESULTS Eighty eight of 121 strains were Gram negative bacilli (72.7%). The predominant pathogens were Pseudomonas aeruginosa (24.0%), Staphylococcus aureus (20.7%), Aerobacter aerogens (19.0%), Drug sensitivity showed that resistant bacteria were markedly increasing but most bacteria were sensitive to imipenem, ciprofloxacin and cefperazone/sulbactam. CONCLUSIONS The Gram negative bacilli are the major pathogens in deceased patients with lower respiratory tract infection. The incidence of A. aerogens infection is increasing. The resistant bacteria are increasing. Sputum culture and drug sensitivity should be seriously considered and reasonable application of antibiotics should be emphasized.
