三七、青黛等对溃疡性结肠炎组织中Fas/FasL表达的影响

摘    要:目的:评价清热解毒、活血化瘀药物三七、青黛、马齿苋对溃疡性结肠炎(UC)大鼠免疫细胞介导Fas/FasL细胞凋亡途径的影响。方法:Wistar大鼠随机分为正常组、模型组、三七组、青黛组、马齿苋组、混合组(三七、青黛、马齿苋)、柳氮磺胺吡啶片组(SASP组),并以三硝基苯磺酸(TNBS)诱发大鼠T细胞免疫机制UC模型。造模后各组第3天起开始分别以生理盐水、三七、青黛、马齿苋、混合物(三七、青黛、马齿苋)、SASP,1次/d,共5d。第6d处死,剖取结肠,记录病变组织大体损伤指数,常规病理观察各组抗炎效果,并检测结肠组织中Fas/FasL蛋白表达水平,以及组织中单核细胞趋化因子(MCP-1)。结果:除马齿苋组外,其余各治疗组见不同程度黏膜修复,与模型组相比在组织学分级上有差异。其中三七组、青黛组黏膜大部分较完整,其组织学分级优于马齿苋组和SASP组(P<0.05);用药后三七组下调Fas和FasL蛋白表达的作用最强,其效果优于马齿苋组和SASP组(P<0.05),而与青黛组比较无显著差异(P>0.05);三七组、青黛组、三药混合组的阻断MCP-1水平效应优于马齿苋组和SASP组(P<0.05)。结论:三七与青黛抗炎、阻断炎症细胞向黏膜上皮游走和免疫细胞Fas/FasL途径细胞凋亡方面的作用优于马齿苋,显示了三七和青黛可能通过对免疫炎症细胞的阻断从而发挥治疗UC的作用。同时提示肠道脉络瘀阻和湿热蕴毒是UC发病的主要病理因素,肠道湿热夹瘀证与细胞免疫功能密切相关。

Abstract:Objective: To observe the effects of panax notoginseng,baphicacanthus cusia and portulaca oleracea on Fas/FasL expression in lesions of ulcerative colitis. Methods:115 rats were randomly divided into 7 groups,and received TNBS/Ethanol by clysterizing to establish T cell mediated UC model at first 24 h except normal group. From day 3,the rats were respectively treated with enemas as follows radix notoginseng,indigo naturalis,herba portulacae,three herbs compound,SASP and placebo (NS) for 5 days. Sections of colonic tissue were stained with HE for histopathologic evaluation,as well as the activity index of colonic ulcer was graded by naked eye. ABC-Ellisa assay was used to measure content of MCP-1 in colon. Expressions of Fas/FasL on colonic mucosa were detected by immunohistochemistry. Results: Radix notoginseng group is one of the two best groups in morphologic evaluation also as indigo naturalis group(P < 0.05). The contents of MCP-1 in lesions of colonic mucosa in those two groups,as well as compound group,were lower than the others significantly. The expressions of Fas/FasL in radix notoginseng group and indigo naturalis group were lower than the others, which correlated with pathologic grade and content of MCP-1 significantly(P < 0.05),and there was no significantly defferent between those two groups(P > 0.05). Conclusion:Radix notoginseng and indigo naturalis could both effectively inhabit the Fas/FasL-mediated colonic epithelial apoptosis by reducing activity of immunocyte. Blood stasis and wet heat-toxicity in large intestine had closely relationship with immunocyte respondence.
