MR影像测量在经蝶垂体瘤手术中的作用

摘    要:目的 探讨MRI相关数据测量在单鼻孔经碟垂体腺瘤的手术方面的指导作用.方法 术前对MRI矢状位影像进行测量,数据1:外鼻孔下缘至鞍底前壁中点距离,数据2:下颚与上述测量线的夹角;利用蝶窦喙突部相对固定于中线位置,三者结合确定鞍底手术方向及深度.将所测量的数据应用于手术中.结果 35例外鼻孔下缘至鞍底前壁中点距离为63~103 mm,平均76.85 mm.鼻道下壁与上述测量线的夹角20.6~33.9°,平均27.52°,将所测量的数值应用于术中,所有患者均顺利抵达鞍底切除肿瘤.结论 利用MRI资料,采集实用性数据对患者进行个性化的解剖研究,对手术具有较大指导意义.

Useness of MR imaging measurement in transsphenoidal pituitary tumor resection

Abstract:Objective To investigate the directive function of MRI data measurement in monoendonasal transsphenoidal pituitary tumor resction. Methods Preoperative MRI image measured were made with data 1 :the diatance from inferior external naris to the center of anterior inferior sella turcica and data 2:the angulation composed with maxilla and the data 1, besides the relative fixed anatomic structure of sphenoid coracoid, above of which determined the structure of inferior sella turcica. And the datas of measurement were applied to the operation. Results 35 patients with pituitary tumor had the measurement on MRI image including 63 - 103 mm ( mean 76.85 mm) in data 1 and 20.6 -33.9°(mean 27.52°) in data 2. All patients were reached the inferior sella turcica with the datas and ectomized the tumor successfully. Conclusions The methods could be used in the preoperative majority of patients for individual research of anatomic structure. It would have great directive functions for the operations via approach of the datas collection.
