中老年高血压与高胰岛素血症关系的初步研究

摘    要:本文分析45例高血压病空腹血糖,血清胰岛素和C肽水平,与正常血压组比较,血清胰岛素明显升高(P<0.01),胰岛素释放指数也明显增高(P<0.01),胰岛素敏感指数明显低于对照组,(P<0.05)。证实中老年期高血压患者也存在高胰岛素血症和胰岛素抵抗状态。

CONTROLLED ANALYSIS OF HYPERINSULINEMIA IN THE MIDDLE AGED AND OLD PATIENTS WITH ESSENTIAL HYPERTENSION

Abstract:The levels of the fasting blood glucose (BG),serum insulin(IS)and C-peptide(CP) were observed in 45 patients with essential hypertension.Of them 28 cases were male and 17 female,within the age range of 40~80 years (average 61.98). All of them suffered from hypertensive heart disease,simultaneosly complicated by coronary heart disease in 28 cases,by cerebral embolization in 10 cases, and by cerebral hemorrhage in 2 cases.The BG level was 5.18±1.68mmol/L in hypertension group and 4.59±0.71mmol/L in normotension group,the difference was of no statisical significance. The level of IS in hypertension group was significantly higher than in the normotension group (19.56±7.6mmol/L VS 12.94±3. 78mμ/L,P<0.01), but the level of CP in the former was lightly higher than in the latter.The ratio of IS/BG (insulinogenic index) was higher in hypertension group than in normotension group (4. 01±1. 8 VS 2.85±0.82,P<0.01), while the ratio of BG/IS (insulin-sensitive index) was notably lower in the former than in the latter (0.31±0.16 VS 0.39±0.14,P<0.05). These results suggest that in essential hypertensive subjects ① there are hyperinsulinemia and insulin resistance; ② the insulinogenic index elevates and the insulin-sensitive index declines;③ the rate of clearance of CP incrcases.
