摘    要:目的:探讨对医院临床手术科室工作绩效的动态综合评价。方法:建立反映主要医疗指标完成情况、医疗指标同比增幅,以及手术工作负荷与效率、出院病历归档时效4个方面的绩效指标体系,采用秩和比法(RSR)对某院8个临床手术科室2008~2009年上半年的绩效进行动态的加权综合评价。结果:①手术科室2009年上半年与2008年同期相比,医疗指标计划完成情况、同比增幅以及手术负荷与效率3个方面的总体水平基本相当(P值均>0.05),出院病历归档时效明显提升(t=5.4331,P=0.0010),综合4个方面的总体绩效水平无明显变化(t=0.0531,P=0.9592);②神经外科综合绩效明显提升,骨科综合绩效显著下降(P值均<0.05);③胸外科、妇产科和眼科的综合绩效一直处于较低水平。结论:针对临床手术科室的工作特点,建立多维的评价指标体系,对其医疗工作绩效进行动态的综合评价,有助于客观地评价临床手术科室工作绩效和临床科主任的岗位职责履行情况,在医院管理中有实用价值。

Abstract:Objective: To explore the method to evaluate the performance of surgery department compre- hensively. Methods: The Rank Sum Ratio (RSR) method was adopted to evaluate comprehensively the weighted performance defined by the accomplishment and the increment of main medical indexies, the quantity and efficiency of operations and the archive duration of the records of eight surgery departments from 2008 to first half of 2009 of one hospital. Results: (1) There were no significient difference of the accomplishment and the increment of main medical i ndexies, the quantity and efficiency of operations between first half of 2009 and 2008(P〉0. 05, respectively). The archive duration of the records was shorter in first half of 2009 than that of 2008(t=5. 4331 ,P=0. 0010). There were no statistical difference of complex performance of upper four aspects between first half of 9,009 and 2008(t=0. 0531,P=0. 9592). (2) The complex performance in first half of 2009 of neurosurgery department was better, orthopaedic department were worse than that of 2008(P〈0. 05, respectively). (3) The complex performance of thoracic surgery , obstetrics and gynecology and ophthalmology department were in low level constantly. Conclusion: It's valuable in evaluating the performance of surgery departments, the achievement of surgery directors and the hospital management to construct a multiple indexes system according the surgery features.
