小剂量氯胺酮复合咪唑安定辅助用于硬膜外麻醉剖宫产

目的：探讨小剂量氯胺酮复合咪唑安定辅助用于硬膜外麻醉剖宫产对母儿的影响。方法：120例在硬膜外麻醉下接受剖宫产的妇女，随机分为对照和试验组，分别给予生理盐水和0．3mg／kg氯胺加0．04mg／kg咪唑安定镇静，观察产妇术中生命体征、牵拉反应和新生儿APGAR评分等。结果：娩取胎儿时，试验组的心率、血压低于对照组，P〈0．05；试验组产妇存娩取胎儿时不舒适反应发生率低于对照组，P〈0．05，部分产妇由于处于睡眠状态而无法早期进行母婴接触，P〈0．05；新生儿APGAR在出生1、2、5min均无统计学差异。结论：小剂量氯胺酮复合咪唑安定辅助用于硬膜外麻醉剖宫产可减轻胎儿娩出时的内脏牵拉反应，对母儿无呼吸循环抑制。

Small - dose Ketamine in Combination with N4idazolam Assisted in Epidural Anesthesia for Cesarean Section

Objective:To study the influence of small-dose of ketamine in combination with midazolam assisted in epidural anesthesia for cesarean section on the neonatal and the maternal.Methods:One hundred and twenty patients undergoing cesarean section with epidural anesthesia were randomly assigned as control group and observed group.In the control group,0.9% NS was given,while those of the observed group received 0.3mg/kg ketamine plus 0.04mg/kg midazolam for sedation.The maternity vital sign,tractive reaction during the operation and the neonatal Apgar scores were observed.Results:When the neonatal was delivered,the heart rate and the blood pressure in the observed group were lower than those in the control group(p<0.05),and the ratio of uncomfortable reaction in the observed group was lower than that in the control group (P<0.05).Partial maternity of study group couldn't go along early mother-baby touch because of being aseep (P<0.05).The neonatal Apgar scores at 1,2,5 minutes had no statistical difference.Conclusion:Small-dose ketamine in combination with midazolam assisted in cesarean section with epidural anesthesia could alleviate the visceral tractive reaction when the neonatal was delivered, which had no respiratory and circulatory inhibition on the neonatal and the maternal.
