雌激素受体、癌胚抗原和DNA含量与胃癌预后关系的研究

用ＡＢＣ免疫组化技术测定６５例胃癌标本的雌激素受体（ＥＲ）和癌胚抗原（ＣＥＡ），并对其中３６例标本进行ＤＮＡ图像分析，对影响预后的２７种因素进行Ｃｏｘ模型分析。结果：胃癌ＥＲ、ＣＥＡ阳性率与浸润深度、淋巴结转移呈正相关，与分化程度呈负相关，ＣＥＡ、ＥＲ阴性的胃癌预后优于ＣＥＡ、ＥＲ阳性的胃癌，ＥＲ阳性胃癌对化疗较敏感。二倍体胃癌预后优于异倍体胃癌，晚期、分化差胃癌细胞核ＤＮＡ含量高。影响胃癌预后的６种因子依次为：病理类型、病程、ＤＮＡ含量、化疗、淋巴结转移和ＥＲ含量。

ESTROGEN RECEPTOR, CARCINOEMBRYONIC ANTIGEN, DNA CONTBNT AND THEIR RELATION WITH GASTRIC CANCER

Estrogen receptor (ER) and carcinoembryonic antigen (CEA) were measured in 65 patients with gastric cancer by Avidin-biotin peroxidase complex (ABC) immunonistochemical technique and the image cytometric (ICM) DNA analysis technique was used to measure the nuclear DNA content in 36 of the 65 patients, 27 parameters which might be relative with prognosis were analyzed by Cox regression model. The results showed that ER and CEA positive rate were positively correlated with the invasive depth, the extent of lymph node metastasis and inversely with the degree of differentiation. The patients with ER-or CEA-positive gastric cancer was more sensitive to chemotherapy than ER-negative one. The patients with diploid gastric cancer had much better prognosis than those with aneuploid gastric cancer.The gastric cancer which have poorer differentiation or in late clinicpathological stages had much great cellular DNA content.6 prognostic parameters of gastric cancer were histopathological types, courses of disease, DNA content, chemotherapy, lymph node metastasis and ER content. The results suggested ER and DNA content of gastric cancer correlates with its prognosis.
