巩膜池在青光眼小梁切除术中的应用观察

目的观察小梁切除术制作巩膜池的术后早期降眼压效果,评价其安全性和有效性。方法原发性青光眼80例90眼,常规制作穹窿部为基底结膜辫,然后在巩膜基底层再进行深层巩膜切除作为房水滤池,剖切深度至90%巩膜厚度,最后切除小粱组织,联合外路可拆除式调整缝线缝合巩膜辫,分层缝合筋膜和结膜。结果应用巩膜池治疗青光眼80例90眼中与术前相比,术后1周内有30眼眼压6～10mm Hg,45眼眼压11～20mm Hg,15眼>21mm Hg;视力提高83眼(92.2%),术后视力不变6眼(6.7%),术后视力下降1眼(1.1%)。术后2年有82眼眼压在20mm Hg以下。结论巩膜池联合小粱切除术治疗原发性青光眼在术后早期可有效降低眼内压,手术安全有效。

Observation of sclera pool trabeculectomy surgery

Objective To observe the effect of production of scleral trabeculectomy pool postoperative IOP-lowering,to evaluate the safety and effectiveness.Methods Totally 80 cases of 90 primary glaucoma,regular production of limbal conjunctival basement braid,and then further in the basal layer of sclera deep sclerectomy as a water filter housing,sectioned to 90% depth scleral thickness,and finally removal of a small beam organization,joint adjustment of exposed removable braided sutures sewn round the sclera,layered suture fascia and conjunctiva.Results Totally 80 cases of scleral pool glaucoma 90 eyes,compared with the preoperative and postoperative intraocular pressure within 1 week of 30 eyes 6～10mm Hg,45 intraocular pressure 11 ～ 20mm Hg,15 eyes> 21mm Hg,visual acuity improved in 83(92.2 %),postoperative visual acuity unchanged 6(6.7%),postoperative visual acuity decreased one(1.1%),postoperative intraocular pressure within 2 years of 82 eyes below 20mm Hg.Conclusions Scleral pool combined with low-beam in treatment of primary glaucoma surgery can be effective in reducing early postoperative intraocular pressure,surgery is safe and effective.
