外固定支架结合克氏针内固定治疗儿童A2型桡骨远端骨折

目的探讨采用外固定支架结合克氏针内固定治疗儿童A2型桡骨远端骨折的治疗方法及临床疗效。方法自2008年9月～2011年3月采用外固定支架结合克氏针内固定治疗儿童23-A2型桡骨远端骨折共18例,术后随访,在随访中根据改良Gartland和Werley功能评分系统标准进行评分以观察疗效。结果 18例桡骨远端骨折均愈合,临床愈合时间为9.0～13.0周,平均11.3周。18例均获得随访,随访13.0～42.0个月,平均24.6个月,在最终的随访中根据改良Gartland和Werley功能评分系统评分0～2分10例,3～8分8例,优良率为100%。结论对于年少的儿童桡骨远端A2型骨折患者,采用采用外固定支架结合克氏针内固定治疗具有手术时间短、术后骨折愈合快、术后并发症少等优点,值得在临床工作中进一步推广运用。

External fixation combined with Kirschner wire internal fixation for distal radius fracture(Type A2) in children

Objective To explore the clinical efficacy of external fixator and K-wire fixation for the treatment of children with type A2 distal radius fracture.Methods From September 2008 to March 2011,18 children with type A2 distal radius fracture were subjected to external fixator and K-wire fixation.During the postoperative follow-up period,the clinical efficacy was assessed according to the modified Gartland and Werley functional scoring system.Results Eighteen cases of distal radius fractures were healed with the healing time being 9.0 to 13.0 weeks(mean 11.3 weeks).All 18 cases were followed up for 13.0 to 42.0 months(mean 24.6 months).According to the modified Gartland and Werley functional scoring system,10 cases obtained 0 to 2 points and 8 cases 3 to 8 points for 8 cases.The excelent and effective rate was 100%.Conclusion For the children with type A2 distal radius fractures,the external fixator and K-wire fixation therapy after surgery has a shorter operative time,quicker postoperative fracture healing,less complications.
