45岁以下子宫内膜癌患者的诊断与治疗探讨

目的探讨45岁以下子宫内膜癌诊断与治疗。方法回顾分析2005年1月至2010年12月≤45岁子宫内膜癌19例病例资料,与同期>45岁的患者126临床资料作对比。结果本组19例占同期收治子宫内膜癌的13.1%(19/145),其中6例未孕,占31.5%,临床表现为阴道流血、排液、月经量多经期延长;18例诊刮确诊,1例外院因功血行子宫切除诊为内膜癌,需作补充手术住院;结论 18例手术治疗,1例孕激素治疗后促排卵妊娠,现已足月分娩。结论 45岁以下年轻妇女子宫内膜癌临床期别较早,病理类型主要为子宫内膜样腺癌,细胞分化、肌层侵润,差异无统计学意义。治疗以手术为主,对高危患者辅以放、化疗,对于有生育要求低危患者,可保留其生育功能。

Diagnostic and therapeutic discuss of endometrial carcinoma patients aged 45 years and younger

Objective To investigate the characteristic and treatment of endometrial carcinoma who under 45 years old.Methods Retrospective study of 19 cases in our hospital from Januray 2005 to October 2010.Results They all have irregular menstruation,mainly are adenocarcinoma,18 of them take operation,only 1 give birth to a child after take Progestogen therapy.Conclusions The patient of endometrial cancer who under 45 years old are rare,mostly they are in early stage,mainly are adenocarcinoma,operation is the common therapy,they can keep biological reproductive function for the women they want to be pregnant later.But the cases with high risk factors need accept the radiation therapy or chemotherapy.
