鼻内镜术后不同鼻腔填塞材料的效果分析

目的:探讨和研究鼻内镜术后不同鼻腔填塞材料对鼻腔恢复的效果。方法:选择我院2008年1月-2010年12月120例鼻腔和鼻窦病患者,均在鼻内镜下施行手术,其中男性90例,女30例,年龄在(20～70)岁,平均年龄35岁,鼻中隔成形术有65例,其中45例时施行了下鼻甲部分切除术,120例患者按鼻腔填塞材料不同分为四组,30例采用凡士林纱条填塞的患者为凡士林纱条组;30例采用凡士林纱条+胶原蛋白海绵填塞的患者为凡士林纱条+胶原蛋白海绵组;30例采用Merocel填塞的为Merocel组;30例采用藻酸钙填塞的患者为藻酸钙组,对四组患者舒适情况、出血情况、鼻部症状、填塞损伤情况进行对比分析。结果:术后鼻腔填塞疼痛程度,①凡士林组:不适感12例,术后出血11例,鼻部症状有轻微胀痛,鼻腔粘膜损伤。②胶原蛋白海绵组:手术后不适有10例,术手出血8例,鼻部轻痛,并出现少量出血。③Merocel组:手术后不适有6例,术后出血6例,鼻部出现轻痛2例,鼻部出现轻度充血。④藻酸钙组:术后出现不适有7例,术后出血8例,鼻部正常,粘膜微损。结论:填塞材料各有所长,在对鼻腔和鼻窦病患者手术中应根据患者个体情况选择相应的材料及恰当的填塞方法,以减少患者的出血时及减轻疼痛。

Effectiveness Analysis of Different Nasal Packing Materials after Endoscopic Sinus Surgery

Objective:To explore and study of endoscopic sinus surgery nasal packing material recovery of the nasal cavity effect.Methods:January 2008 December ~2010 120 cases of nasal cavity and paranasal sinuses in patients with intranasal endoscopic surgery,90 cases were male and 30 females,aged 20-70 years old,average age 35,nasal septum plasty 65 cases,of which 45 cases underwent partial resection of the inferior turbinate,120 cases of patients with nasal packing material is divided into four groups,30 patients were treated with vaseline gauze group for the vaseline;30 vaseline + collagen sponge filled patients were treated with vaseline + collagen sponge group;30 cases with Merocel packing Merocel group;30 cases of patients with the packing of the alginate is calcium alginate group,four groups patient comfort,bleeding,nasal symptoms,packing damage were analyzed.Results: postoperative nasal packing degree of pain the ① Vaseline groups: discomfort in 12 cases,11 cases of postoperative bleeding there was a slight pain,nasal symptoms,nasal mucosal injury.The the ② collagen sponge group: post-operative discomfort of 10 cases,surgery hand bleeding in 8 cases of mild pain,nose,and a small amount of bleeding.③ the Merocel Group: post-operative discomfort with six cases,six cases of postoperative bleeding,nose light pain in 2 cases of mild congestion,nose.④calcium alginate group: 7 cases of postoperative discomfort,postoperative bleeding in 8 cases,normal nose,mucous membrane micro-damage.Conclusion:filling materials have their own strengths,in the nasal cavity and paranasal sinuses in patients with surgery should be based on individual patient circumstances to select appropriate materials and proper packing methods to reduce bleeding in patients with and alleviate pain.
