64层螺旋CT容积扫描各种后处理技术在腰椎峡部裂诊断中临床价值分析

摘    要:目的探索64层螺旋CT容积扫描的各种后处理技术在腰椎峡部裂诊断中的临床意义。方法应用64层螺旋CT对38例腰椎峡部裂患者进行容积扫描,对获得的图像进行多平面重建(MPR)、容积显示技术(VR)及曲面重建(CRP)等后处理技术,观察腰椎峡部裂显示情况以及椎体的滑脱情况,计算各种检查方式的显示率,筛选出最佳的重建方式。结果对38例患者腰椎的容积扫描图像及后处理图像进行分析,共发现腰椎峡部裂74处,其中Ⅰ度滑脱28例,Ⅱ度滑脱6例,Ⅲ度滑,1例,无滑脱3例;在各种后处理技术中,薄层最大密度投影CPR对腰椎峡部裂以及腰椎滑脱的显示率最高,为100%。结论64层螺旋CT容积扫描及后处理技术在腰椎峡部裂以及腰椎滑脱诊断中具有较高的应用价值,薄层最大密度投影CPR重建诊断价值最高。

Abstract:Objective To explore the value of 64-slice spiral CT with various processing in the diagnosis of spondylolysis of lumbar spine.Methods 38 cases with lumbar spondylolysis patients were examined by 64-slice spiral CT,The data was transmitted to workstation and the images were reconstructed with the techniques of multiple planar reconstruction(MPR),volume rendering technique(VR) and surface reconstruction(CRP)in order to observe the lumbar spondylolysis and vertebral slip,calculate display rate,screen out the best ways of reconstruction.Results 74 lumbar spondylolysis were found in 38 cases,which were found by 64-slice spiral CT and the image reconstruction technique.In the cases 28 cases were the I degree slip of upper lumbar,6 cases were the II degree slip of upper lumbar,1 case was the III degree slip of upper lumbar;3 cases were no slippage.In various post-processing technology,maximum intensity projection CPR on lumbar spondylolysis and spondylolisthesis showed the highest rates,are 100%.Conclusion the 64 layer spiral CT volume scanning and postprocessing techniques has higher application value in the diagnosis of lumbar spondylolysis and spondylolisthesis,and maximum intensity projection CPR have highest diagnostic value.
