颅脑外伤患者高压氧介入时机及效果探讨

目的:探讨高压氧治疗颅脑外伤患者的最佳介入时机,并观察其临床治疗效果及应用价值。方法:回顾性分析90例颅脑外伤患者的一般资料,按照病程的长短、对照的原则分为A组和B组,各为45例。A组患者为伤后10d～1个月,B组患者为伤后1～2个月,两组均采用高压氧进行治疗,采用Glasgow昏迷量表(GCS)和严重颅脑外伤残疾评定量表(DRS)对两组患者治疗前、治疗4周进行评定,并进行统计学分析。结果:两组患者治疗前GCS评分、DRS评分比较,差异均无统计学意义(P>0.05),治疗4周后,两组患者GCS评分均得到提高,DRS评分均有所下降,差异显著(P<0.01),但A组患者GCS评分提高更为明显,DRS评分下降更为明显,与B组比较,差异具有统计学意义(P<0.01)。结论:高压氧治疗颅脑外伤患者的最佳介入时机为伤后1月以内,能够有效的改善患者的病情及预后,临床治疗效果远远优于超过1个月的治疗,值得临床进一步大胆推广应用。

Patients with Craniocerebral Injury in Time and Effect Explore Hyperbaric Oxygen

Objective:Explore craniocerebral trauma hyperbaric oxygen therapy in the best time,and observed the clinical treatment effect and application value.Methods:Craniocerebral injury were retrospectively analyzed 90 cases of patients with general data,according to the length of the course,control principle and divided into A group and group B,each for 45 patients.A group of patients after injury 10 d ~ 1 months,group B patients after injury for 1 to 2 months,two groups are treated with hyperbaric oxygen,the Glasgow coma scale(GCS) and severe craniocerebral trauma disability rating scale(DRS) to two groups of patients before treatment,treatment 4 weeks to assess,and statistical analysis.Results:Two groups of patients before treatment,DRS GCS score ratings comparison,there were no statistically significant difference(P> 0.05),treatment,four weeks later,two groups of patients are improved GCS score,DRS scores are declined,significant difference(P< 0.01),but A group of patients with GCS score is more apparent,improve the DRS score down is more apparent,and group B,A statistically significant difference compared(P< 0.01).Conclusion:Hyperbaric oxygen therapy in patients with craniocerebral injury in time for the best injury within 1 month,can effective improve a patient’s condition and prognosis,clinical treatment effect is far superior to more than 1 months of treatment,and clinical further bold application.
