增强MRI检查在早期股骨头坏死诊断中的应用

目的探讨增强MRI检查在早期股骨头坏死诊断中的应用价值。方法选取2017年1月至2018年6月我院疑似早期股骨头坏死患者80例,所有患者均行增强MRI以及CT检查,以关节镜检查结果为金标准,分析增强MRI以及CT在早期股骨头坏死中的诊断价值。结果80例疑似早期股骨头坏死患者中,关节镜检查确诊32例为早期股骨头坏死,其中Ⅰ期13例,ⅡA期9例,ⅡB期7例,ⅡC期2例,Ⅲ期1例。增强MRI诊断早期股骨头坏死的准确性为86.25%、灵敏度为81.25%、特异度为89.58%、阳性预测值为83.87%、阴性预测值为87.75%,均显著高于CT的60.00%、53.13%、64.58%、50.00%、67.39%(P<0.05)。结论增强MRI诊断早期股骨头坏死具有较高的准确性、灵敏度、特异度,可为该病的临床诊治提供可靠的影像学依据。

Application of Enhanced MRI Examination in the Diagnosis of Early Femoral Head Necrosis

Objective To explore the application value of enhanced MRI examination in the diagnosis of early femoral head necrosis.Methods 80 patients with suspected early femoral head necrosis from January 2017 to June 2018 in our hospital were selected.All patients were examined with enhanced MRI and CT.With the results of arthroscopy as the gold standard,the diagnostic value of enhanced MRI and CT for early femoral head necrosis was analyzed.Results Among 80 patients with suspected early femoral head necrosis,32 cases were diagnosed as early femoral head necrosis by arthroscopy,including 13 cases in stageⅠ,9 cases in stageⅡA,7 cases in stageⅡB,2 cases in stageⅡC and 1 case in stageⅢ.The accuracy,sensitivity,specificity,positive predictive value and negative predictive value in the diagnosis of early femoral head necrosis of enhanced MRI were 86.25%,81.25%,89.58%,83.87%,87.75%respectively,significantly higher than 60.00%,53.13%,64.58%,50.00%and 67.39%of CT,respectively(P<0.05).Conclusions Enhanced MRI in diagnosis of early femoral head necrosis has higher accuracy,sensitivity and specificity,which can provide reliable imaging basis for the clinical diagnosis and treatment of early femoral head necrosis.
