心房心室双螺旋主动电极在双腔心脏起搏器植入术中的应用效果及安全性

摘    要:目的探讨心房心室双螺旋主动电极在双腔心脏起搏器植入术中的应用效果和安全性。方法选取86例病态窦房结综合征、房室传导阻滞患者,随机分为A、B组各43例。A组给予心房心室双螺旋主动电极植入治疗,B组给予心房被动加心室主动单螺旋电极植入治疗。比较两组的电极植入情况、手术指标、术后指标。结果两组的心室、心房电极一次成功植入率比较,差异无统计学意义(P>0.05);B组2例在缝合固定心房电极时出现脱落,需要重新植入。两组患者的X线暴露时间、总手术时间比较,差异无统计学意义(P>0.05);A组的术后卧床时间显著短于B组(P<0.05)。两组的术后随访时间、术后并发症发生率比较,差异无统计学意义(P>0.05)。结论心房心室双螺旋主动电极应用于双腔起搏器植入术中,心室、心房电极脱位率低,并且不增加手术时间、X线暴露时间,还能缩短患者术后卧床时间。

Abstract:Objective To explore the application effect and safety of atrial ventricular double-spiral active electrodes in dual-chamber pacemaker implantation.Methods 86 patients with sick sinus syndrome or atrioventricular block were selected and randomly divided into two groups equally.Group A was treated with atrial ventricular double-spiral active electrodes implantation,while group B was treated with atrial passive plus ventricular active single spiral electrode implantation.The electrode implantation,surgical indicators and postoperative indicators were compared between two groups.Results No significant difference was found between two groups in the one-time successful implantation rates of ventricular and atrial electrodes(P>0.05),while 2 cases in group B had fall-off of atrial electrode when suture and fixation and needed to be implanted again.No significant difference was found between two groups in the X-ray exposure time and total operation time(P>0.05).The postoperative bed rest time of group A was significantly shorter than that of group B(P<0.05).No significant difference was found between two groups in the postoperative follow-up time and incidence of postoperative complications(P>0.05).Conclusions Atrial ventricular double-spiral active electrodes applied in dual-chamber pacemaker implantation has low dislocation rates of ventricular and atrial electrodes,does not increase the operation time and X-ray exposure time,and can shorten the postoperative bed rest time.
