Hp与胃粘膜病变——20年前后比较

目的:为了了解20年前Hp有无感染以及感染率和感染与胃粘膜病变的关系,再结合20年后感染情况的比较,为进一步研究Hp与胃粘膜病变提供依据。方法:将1973年563例胃镜活检材料与同期1992年500例胃镜活检材料进行回顾性病理切片的阅读,记录Hp感染情况及胃粘膜病变,然后进行对比分析。结果:1973年胃粘膜活检中Hp感染率为71.0％,1992年为67.2 ％,两者几乎相等。说明在1986年以后才认识到的Hp感染早在20年前已普遍存在。进一步对慢性浅表性胃炎、慢性萎缩性胃炎、胃溃疡、十二指肠球溃疡、胃癌5种主要胃粘膜病变中Hp感染率20年前后比较亦无大的变化。结论:胃粘膜感染Hp是一个历史长久的问题,可能会追溯到古代,因此,对Hp与胃粘膜病变的内在关系要进行深入的研究,只凭短期内的材料就做出Hp与胃炎、胃癌有关的结论应取慎重态度的。

Comparative study on Heli cobacter pylori (Hp) associated gastric mucosa twenty years interval

AIM The differences of Helicobacter pylori positive rate for two groups with twenty years interval and the correlations with different gastric mucosa lesions were studied.METHODS A retrospective study was performed on 563 in 1992.These gastric biopsies in 1973 and 500 in 1992.These five groups:those with chronic superficial gastritis,those with chronic gastritis,those with gastric ulcer;those with duodenal ulcer and those with gastric carcinoma.RESULTS Helicobacter pylori was detected in 400 cases of 563 cases(71.0 %)and on 366 cases of 500 cases(67.2%).The gastric epithelium colonized by Helicobacter pylori showed a characteristic changes,including loss of apical mucous portion of individual cells,drop-out of epithelial cells,erosion for the two groups This results showed the Helicobacter pylori infection had few changes for the twenty years.The positive rate of Helicobacter pylori in the five groups of gastric lesions-chronic superficial gastritis,chronic at-rophic gastritis,gastric ulcer,duodenal ulcer and gastric carcinoma,also showed few changes for the twenty years.CONCLUSIONS Helicobacter pylori infection in gastric mucosa has a same results for the two groups in 1973 and in 1992 in Beijing,China From view of this point,the relationship between the Helicobacter pylori and gastric mucosa lesions has a long history and may be back to ancient time for human being.Therefore,the next more research for the helicobacter pylori in the pathongenesis of gastric lesions need to be performed widely.
