凝血指标在慢性肝病中的改变及其意义

目的观察年龄和性别因素对凝血指标的影响，并探讨在排除年龄和性别因素的干扰后，慢性肝炎或肝硬化患者的凝血指标发生的变化。方法观察性别、年龄因素对88例未用促凝药物外伤患者凝血酶原时间（PT）、部分凝血酶原时间（APTT）和凝血酶时间（TT）的影响。以未用促凝药物外伤患者88例作为对照，检测55例慢性乙型肝炎患者和67例肝硬化患者在排除性别、年龄干扰因素的影响后，凝血指标（PT、APTT和TT）发生的改变。结果高年龄组（60—80岁）人群PT显著比其他人群长，APTT显著比他人群短；女性人群APTT显著比男性人群APTT长。和对照组比较，慢性乙型肝炎和肝硬化患者PT、APTT、和TT显著延长。结论在排除年龄和性别影响后，慢性乙型肝炎和肝硬化患者仍然存在出血倾向；TT是反映凝血功能损伤程度好的指标。

The changes of coagulant index in chronic liver diseases

Objective To explore the changes of coagulant function in patients with chronic hepatitis and hepatocirrhosis with or without the inclusion of the factors of age and sex. Methods The influences of sex and age on the prothrombin time(PT),apart prothrombin(APTT) and thrombin time(TT) were defined in 88 wounded patients without the administration of haemostatic.PT,APTT and TT were determined in 55 patients with hepatitis B and 67 patients with hepatocirrhosis after the influences of age and of sex were excluded. Results PT in patients aged from 60 to 80 was significantly longer than other groups.APTT of female group was significantly longer than that of male group.PT,APTT and TT in group of patients with hepatitis B and in group of patients with hepatocirrhosis were significantly longer than that of control. Conclusion Our study shows that hepatitis B and hepatocirrhosis have a significant influence on patients' coagulant function even the factors of sex and age are excluded. TT,in our opinion,is the best index reflecting the losing degree of coagulation.
