周剂量艾素联合氟尿嘧啶及顺铂治疗晚期胃癌31例

目的观察周剂量多西紫杉醇（艾素，TAT）联合亚叶酸钙／5-氟尿嘧啶（CF／5-Fu）及顺铂（DDP）治疗晚期胃癌的临床疗效与不良反应。方法31例晚期胃癌患者接受TAT与CF／5-Fu及DDP联合化疗：TAT 35mg／m^2，静滴1h，d1、8、15；DDP25mg／m^2，静滴d1、8、15；CF 100mg，静滴2h，d1、8、15；5-Fu 750mg／m^2，24h微泵持续静滴，d1、8、15；28d为一个周期。治疗2个周期后评价疗效和不良反应。结果31例患者均可评价疗效，有效率51．6％。初治患者有效率为63．6％（其中2例CR），复治患者有效率为22．2％。中位疾病进展时间6个月，中位生存期11个月，1年生存率41．9％。主要不良反应为骨髓抑制、胃肠道反应及脱发。结论周剂量艾素联合亚叶酸钙／5-氟尿嘧啶及顺铂治疗晚期胃癌缓解率高，毒副反应可以耐受。

Daetxel Given Once Weekly Combined with Cisplatin and 5 - Fluororacil to Treatment Advamced Gastric Cancer Patients in 31 Cases

Objective To observe the effects and toxicity of docetaxel given once weekly combined with cisplatin and 5 - fluororacil in the treatment of advanced gastric cancer（AGC）. Methods Thirty - one patients with AGC were given docetaxel at 35 mg/m^2 square by intravenous infusion completed in 1 hour,and cisplatin was given at 25mg/m^2 square for 4 hours ,5 -fluororacil was also given at 750 mg/m^2 square for 24 hours on the 1 st,8th and 15th days, respectively. One treatment cycle lasted for 28 days and 2 consecutive cycles were administered. Results In the 31 patients available for therapeutic effect evaluation, the overall effective rate was 51.6%. The effective rate in patients without previous treatments was 63.6% （ CR in 2 cases） ,and 22. 2% in the retreated patients. The median TTP,median survival time and one - year survival rate were 6 months, 11 months and 41.9%, respectively. The main toxic reactions of the combined treatment were myelosuppression, gastrointestinal tract reaction and alopecia of mostly grade 1 or 2.
