疏肝利胆法治疗胆道结石术后胆道动力障碍的研究

目的探讨疏肝利胆法——“胆通颗粒”对中药治疗胆囊切除术后胆道动力障碍（PCBD）的价值。方法选择20002～2006年临床资料完整，出现PCBD的病例60例，将其随机分2组，对照组30例采用常规治疗；治疗组30例口服胆通颗粒。通过“T”型管或改良ERCP导管测定服药1W后SOBP、SOCA、SOF、SOCD、CBDP及观察临床症状改变来研究其对恢复胆道正常功能的作用。结果对照组SOBP、SOCA及临床症状无明显降低和改善；治疗组SOBP、SOCA明显降低，临床症状明显改善，组内、组间比较有显著差异（P〈0．01）。结论应用疏肝利胆法——“胆通颗粒”能够尽快恢复胆道的正常运动，对治疗术后PCBD的发生有明显的作用。

Research on PCBD after Treating Biliary Calculi with Liver-Dispersing and Bile-Discharging Therapy

Objective： To probe into the value of dispersing the liver and discharging the bile theropy Dan Tong granules for treating PCBD after cholecystectomy. Methods： Sixty PCBD cases which have completely clinical analysis from 2002 - 2006 were divided into two groups in random. The control group of 30 cases were treated with conventional therapy only. 30 cases in the treatment group were administrated Dan Tong granules. The effects on recovering normal biliary function were studied by using T tube or modified ERCP duct to determine SOBP, SOCA, SOF, SOCD and CBDP after a week and the change of clinical symptoms was observed. Results： The SOBP, SOCA and clinical symptoms had no obvious improvement in the control group. SOBP and SOCA of the therapeutic group decreased greatly and the clinical symptoms improved significantly. There were significant differences in each group and between two groups. Conclusion： Liver - dispersing and bile - discharging Dan Tong granules can recover the normal motion of biliary tract . It has a significant effect on treating the PCBD after operation.
