家兔肩关节囊打褶缝合模型的建立及初步组织学研究

摘    要:目的 建立家兔肩关节囊打褶缝合的实验动物模型,探讨此模型的可行性并进行初步的关节囊组织学研究. 方法选取18只成年雄性日本大耳白兔,对其进行切开下肩关节后关节囊打褶缝合术.分别于术后1 d、3 d、1周、2周、4周、6周各处死3只白兔,观察术后伤口感染、打褶失败等相关并发症的发生率,评价动物模型的可行性;同时对术后肩关节囊标本进行切片及HE染色,观察家兔正常肩关节囊的组织结构及打褶术后早期关节囊的组织病理变化;对比家兔与人的肩关节囊组织结构. 结果家兔肩关节囊打褶缝合的手术入路相关解剖位置比较恒定,手术操作过程可重复性高.术后无伤口感染发生,有1只(术后3 d)打褶失败,发生率为5.6%.无非预计性死亡发生.家兔肩关节囊的组织结构与人相似,可分为滑膜层、滑膜下层及纤维层.家兔关节囊打褶术后最早于术后第1天即可见到炎症细胞浸润、新生毛细血管形成等病理变化. 结论论证了应用家兔制备肩关节囊打褶缝合模型的研究可行性,家兔具有与人相似的肩关节囊组织结构,打褶术后最早于术后第1天即可出现炎症细胞浸润、新生毛细血管形成等病理变化.

A histological study of capsular plication of shoulder joint In a rabbit model
Abstract:Objective To establish an animal model of shoulder capsular plieation in rabbits and observe the histological changes in the capsules. Methods Open capsular plication of posterior wall of shoulder joint was performed in each of the 18 male mature Japanese Rabbits. The rabbits were sacrificed at 1 day, 3 days, 1 week, 2, 4 and 6 weeks postoperatively. Each time 3 were killed. Postoperative complications such as infection and failure of capsular plication were observed to evaluate the feasibility of the animal model. Histological observations of the normal capsular tissue and the capsules at the early postoperative stage were done with HE sections. Results This animal model provided a stable surgical approach which ensured a highly replicable procedure. No infection or unpredictable death was observed postoperatively. There was only 1 case of plication failure at 3 days postoperatively (5.6%). The shoulder joint capsule of rabbit was histo-logically similar to that of human, which could be divided into synovial, suhsynovial and fibrous layers. Pathological changes such as inflammatory infiltration and neevascularization after capsular plication could be found as early as just 1 day postoperatively. Conclusions The rabbit model of shoulder capsular plication is feasible. The rabbit shoulder joint capsule is histologically similar to that of human. Pathological changes such as inflammatory infiltration and neovascularization can occur soon after capsular plication.
