柴胡龙骨牡蛎汤加减配合颈椎牵引治疗颈性眩晕

目的观察柴胡龙骨牡蛎汤配合颈椎牵引治疗颈性眩晕的临床疗效。方法选择60例颈性眩晕患者，采用区组随机化方法及异体配对设计分为治疗组和对照组，每组各30例。治疗组予柴胡龙骨牡蛎汤配合颈椎牵引治疗，对照组予盐酸氟桂利嗪胶囊配合颈椎牵引治疗，治疗4周进行疗效评估，两组治疗前后均对患者进行症状与功能评分及经颅多普勒超声检测仪检查。结果两组症状与功能评分情况：治疗组治疗后症状与功能评分与治疗前比较显著提高（t=2．641，P〈0．01），对照组治疗后症状与功能评分与治疗前比较也有提高（t=1．905，P〈0．05）；两组治疗后比较，治疗组明显高于对照组（t=2．183，P〈0．05）。两组TCD显示：治疗组治疗后椎动脉（VA）及基底动脉（BA）的平均流速（Vm）与治疗前比较均明显提高（t=2．659，P〈0．01）j血管搏动指数（PI）明显降低（t=2．587，P〈0．01），对照组治疗后VA及BA的Vm与治疗前比较也有提高（t=1．716，P〈0．05），PI降低（t=1．805，P〈0．05）；两组治疗后比较，治疗组的VA及BA的Vm均高于对照组（t=2．067，P〈0．05），治疗组的VA及BA的PI均低于对照组（t=2．136，P〈0．05）。治疗组优良率为90％，对照组优良率为60％，两组差异有统计学意义（x2=43．01，P〈0．05）。结论柴胡龙骨牡蛎汤加减配合颈椎牵引能明显改善颈性眩晕患者的临床症状，提高生活质量。

ChaihujiaLongguMuli decoction combined with cervical traction in the treatment of cervical vertigo

Objective To observe the clinical efficacy of ChaihujiaLongguMuli decoction combined with cer- vical traction in the treatment of cervical vertigo. Methods 60 patients with cervical vertigo were divided into treat- ment group and control group by blocked randomization method and homologous pairing design, 30 cases in each group. Treatment group was given ChaihujiaLongguMuli decoction combined with cervical traction therapy. The control group received flunarizine hydrochloride capsules combined with cervical traction therapy. The clinical efficacy was as- sessed after treatment for 4 weeks. The symptom and function score and transcranial Doppler （TCD） were examined before and after treatment. Results After treatment, the symptom and function score of the two groups were signifi- cantly increased compared with before treatment. After treatment, the symptom and function score of the treatment group was significantly higher than that of control group （t = 2. 183, P 〈 0.05 ）. The results of TCD showed that, the Vm of VA and BA after treatment in two groups were significantly increased, PI was significantly decreased compared with before treatmentl After treatment, Vm in VA and BA of the treatment group were significantly higher than those ofthe control group （ t = 2. 067 ,P 〈 0.05 ）. PI in VA and BA of the treatment group were lower than those of the control group（t = 2. 136, P 〈 0.05 ）. The excellent and good rate was 90% in treatment group, that was 60% in control group, the difference between the two groups was significant （ X2 = 43.01, P 〈 0.05 ）. Conclusion ChaihujiaLonggu- Muli decoction combined with cervical traction can effectively relieve the clinical symptoms of the patients with cervi- cal vertigo, improve quality of life of the patients.
