腹腔镜治疗巨大卵巢良性肿瘤的临床优势

目的探讨腹腔镜治疗巨大卵巢良性肿瘤的可行性及优势。方法回顾性分析腹腔镜下巨大卵巢良性肿瘤手术20例（腹腔镜组）与开腹巨大卵巢良性肿瘤手术（开腹组）30例的临床资料,比较2组的手术时间、出血量、住院时间、术后恢复情况等。结果 2组患者均顺利完成手术。术中腹腔镜组与开腹组平均手术时间分别为（48.0±12.8）、（68.5±22.6）min,术中出血量分别为（27.2±18.6）、（60.0±29.4）mL,平均住院日分别为（3.00±1.1）、（3.86±1.7）d,2组比较差异有统计学意义（P〈0.05）;术后排气时间腹腔镜组与开腹组分别为（24.1±2.5）、（30.2±4.7）h,2组比较差异有统计学意义（P〈0.05）;2组患者术后发热率腹腔镜组为10%,开腹组为27%,2组患者比较差异无统计学意义（χ2=3.1,P〉0.05）。结论腹腔镜治疗巨大卵巢良性肿瘤临床可行,具有创伤小、手术时间短、术中出血少、术后住院时间短等优势。

Clinical advantages of laparoscopy in the treatment of large benign ovarian cysts

Objective To investigate the feasibility and advantages of laparoscopic treatment for large benign ovarian cysts. Methods The clinical data of 20 cases with large benign ovarian cyst under laparoscopic surgery（ laparoscopic group） and 30 cases with ovarian cyst under transabdominal benign ovarian cystectomy（ laparotomy group） were retrospectively reviewed. Results The operation time of the laparoscopic group was shorter（ 48. 0 ± 12. 8） min than that in the laparotomy group（ 68. 5 ± 22. 6） min,there were significant differences between the two groups（ P 0. 05）. The intraoperative hemorrhage volume was significantly less in the laparoscopic group（ 27.2 ±18.6） mL than that in the laparotomy group（ 60.0 ±29.4） mL（ P 0.05）. The length of stay of the laparotomy group（ 3.86 ±1.7） d was significantly longer than that in the laparoscopic group. About the time of gas passage by anus,the laparoscopic group occupied the shorter time（ 24. 1 ± 2. 5） h than that in the laparotomy group（ 30. 2 ± 4. 7h）（ P 0. 05）. Conclusion Laparoscopic treatment of large ovarian cysts has many advantages,such as minimal invasive,less pain,shorter operative time,less blood loss,shorter length of stay and so on.
