体外循环心脏手术中常规超滤技术的应用

摘    要:目的 探讨分析在体外循环心脏手术中使用常规超滤技术的临床应用价值.方法 对57例患者在体外循环转流平稳或复温时行常规超滤,对比分析超滤前后血球压积(Hct)、血浆蛋白和血钾浓度的变化.结果 全组病例超滤出液体量200~2 500 mL,平均(1 338.06±304.31 )mL;经超滤后Hct和血浆蛋白均有明显提高(P<0.05),部分高钾的患者血钾也在短时间下降(P<0.05).结论 在体外循环心脏手术中使用常规超滤技术可快速超滤出体内多余水分及电解质,避免血液过度稀释,使Hct、血浆蛋白和血钾等维持在合适的水平.这项技术适用于各种体外循环心内直视手术,为提高心脏外科手术安全性提供了重要保证.

Abstract:Objective To investigate the effect of ultrafiltration in cardiopulmonary bypass in cardiac surgery. Methods Con- ventional ultrafihration was performed in cardiopulmonary bypass in 57 cardiac surgery. The hematocrit (Her), plasma protein and serum potassium concentration changes were compared before and after uhrafihration. Results The ultrafiltration volumes were 200 - 2 500 ( 1 338.06 ± 304.31 ) ml. The Het, plasma protein increased significantly and the serum potassium concentration decreased ( P 〈 0. 05 ) after ultrafiltration. Conclusion Ultrafiltration can quickly filter out the supernumerary water, concentrate the blood and maintain the normal serum potassium concentration. It is useful to improve the security of operations.
