选择性痔上黏膜切除术与吻合器痔上黏膜环切术治疗重度内痔的疗效对比分析

摘    要:为比较选择性痔上黏膜切除术(TST)与吻合器痔上黏膜环切术(PPH)治疗重度内痔的临床疗效,将576例内痔患者随机分为观察组和对照组,各288例。观察组采用TSI、治疗,对照组采用PPH治疗,对比观察两组治愈率、术后住院时间、住院费用、患者满意度,以及术后剧烈疼痛、出血、尿潴留、肛门坠胀感、吻合口狭窄等并发症的发生情况。结果显示,两组治愈率、术后住院时间、住院费用、患者满意度、术后剧烈疼痛发生率差异无统计学意义,P〉0.05;但观察组术后尿潴留、出血、肛门坠胀感、吻合口狭窄发生率明显低于对照组,P〈0.05。结果表明,TST和PPH治疗重度内痔的总体疗效相当,但在术后并发症方面TST明显优于PPH。

Comparative Study on the Therapeutic Effects of Tissue-selecting Therapy and PPH in the Treatment of Severe Internal Hemorrhoids

Abstract:In order to compare the clinical effects of tissue-selecting therapy(TST) and PPH in the treat- ment of severe internal hemorrhoids, the 576 cases of this disease were randomly divided into observation group( n =288) and control group( n = 288); observation group received TST, control group received PPH ; the curative rate, postoperative hospitalization time, hospitalization cost, patient's satisfactory degree, and the incidence of postoperative complications such as agony, bleeding, uroschesis, anastomosis stoma ste- nosis,and the tenesmus sensation of anus,etc were observed and compared between both groups. As re- sults,in first 4 items of above indexes, as well postoperative agony there was no statistical difference be- tween both groups( P 〉0.05), but in the last items of above indexes observation group was significantly lower than control group( P 〈0.05). Results show that the general curative effects of both procedures are similar, but in postoperative complication observation group was significantly superior tO control group.
