低位肛瘘术后中医辨证分期换药的疗效观察

为探讨低位肛瘘术后中医辨证分期换药的临床疗效，回顾接受手术治疗的113例低位肛瘘患者资料，其中55例患者术后根据不同时期采用大黄油纱、生肌玉红膏油纱换药（治疗组），58例患者采用凡士林油纱换药（对照组）。对比两组患者术后第4、8、14、21天肛门疼痛及肉芽组织生长情况．以及创面愈合时间。结果显示，治疗组术后第4、8、14、21天肛门疼痛评分均日月娩低于对照组，P〈0．05；两组术后第4天肉芽组织生长情况评分差异无统计学意义，P〉0．05，但治疗组术后第8、14、21天肉芽组织生长情况评分均叫显高于对照组，P〈0．05；治疗组创面愈合时间明显短于对照组，P〈0．05。结果表明，根据低位肛瘘术后中医辨证分期采用大黄油纱、生肌玉红膏油纱换药可明显减轻患者肛门疼痛，缩短创面愈合时间，值得临床推广应用。

Efficacies of Staging Dressing Exchange According to the Syndome Differentiation of TCM after Surgery for Low Anal Fistula

In order to explore the clinical efficacies of staging dressing exchange according to the syndome differentiation of TCM after surgery for low anal fistula,author retrospectively analyzed the data of 113 patients with low anal fistula who had received surgery,among whom 55 adopted dressing exchange with rhu- barb oil immersion gauze,or with Shengji Yuhong ointment（a Chinese herb oinment for promoting tissue regeneration）according to different stages（ as treatment group）, 58（as control group） all with petrolatum oil immersion gauze;then on the 4,8,14 and 21 day after operation the pain and granulation tissue growth status, and healing time of wound-surface in both groups were compared. As results, the score ratings on a nal pain,at the 4,8,14,21th day after operation, as well as healing time of wound-surface in treatment group was significantly lower, and shorter than in control group（P 〈0. 05）. There was no statistical difference in the score ratings of granualation tissue growth status at the 4th day after operation between both groups（ P 〈0.05）;but in treatment group at the 8,14,2lth day after operation significantly higher （ P 〈0.05）. Results show that after surgery for low anal fistula staging dressing exchange of treatment group can distinctly reduce anal pain of patients, shorten healing time of wound-surface,thus is worthy to be popularized clinically.
