血清肿瘤标志物的联合检测在胃癌诊断中的价值

目的探讨癌胚抗原（CEA）、糖类抗原（CAl9-9）血清肿瘤标志物在胃癌诊断中的价值，以提高胃癌的诊断率。方法检测50例健康者及50例胃癌患者中的血清CEA、CAl9-9的含量。结果胃癌组CEA水平（59．21±11．84）μg／L和CAl9-9水平（72．16±13．81）U／mL明显高于健康对照组水平（3．57±1．09）μg／L及（10．01±4．03）U／mL，差异具有统计学意义（t=13．197，9．527，均P〈0．05），单项检测在胃癌中的灵敏度和特异性分别为63．2％、52．4％，73．7％，61．9％，两项肿瘤标志物联合检测的灵敏度和特异性分别为86．4％，90．O％。联合检测组灵敏度明显优于肿瘤标志物单项测定。胃癌患者血清CEA和CAl9-9水平有一定的相关性（r=0．346，P〈0．05）。结论CEA和CAl9-9肿瘤标志物联合检测为胃癌的诊断提供可靠依据。

Clinical value of tumor markers combined detection in diagnosis of grastric cancer

Objective To explore the clinical value of carcinoembryonic antigen （ CEA）, CA19-9 in the di- agnosis of gastric cancer, so as to improve the diagnostic rate of gastric cancer. Methods The serum levels of CEA, CA19-9 in 50 healthy subjects（control group） and 50 patients with gastric cancer（ gastric cancer group） were detec- ted. Results The serum levels of CEA （59.21 ± 11.84） μg/L and CA19-9 （72.16± 13.81 ） U/mL in gastric cancer group were significantly higher than those in control group （3.57 ±1.09） i,g/L and （ 10.01 ± 4.03） U/mL, the differences were significant（ t = 13. 197,9. 527, all P 〈 0.05 ）. The sensitivity and specificity of single marker detec- tion was respectively as follows ： CEA （63.2% ,52.4% ）, CA19-9 （73.7% ,61.9% ）. For combination of tumor mark- ers （ CEA ＋ CA19-9 ）, the sensitivity was 86.4%, the specificity was 90.0%. The sensitivity of combined detection was superior to single detection. Correlation analysis showed that CEA and CA19-9 levels was positively correlated in gastric cancer patients （ r = 0.346, P 〈 0.05 ）. Conclusion CEA combined with CA19-9 detection may provide relia- ble evidence for the diagnosis of gastric cancer.
