手术治疗老年人胃癌94例临床分析

目的探讨老年人胃癌的临床特点及手术治疗的疗效。方法回顾性分析94例70岁以上老年胃癌患者的临床资料，分70～75岁组50例，大于75岁组44例。比较两组的术前合并病、术后并发症及病死率。结果术后并发症发生率36．20％，围手术期病死率3．20％，组间比较显示随年龄增大，术前合并症越多及术后并发症越多（P=0．03），但术后病死率差异无统计学意义（P=0．48）。结论高龄胃癌患者虽然手术治疗风险大，但是重视围手术期处理能够使患者耐受手术治疗。

Clinical analysis of the surgery in treatment of 94 elderly patients with gastric cancer

Objective To explore the clinical characteristics of elderly patients with gastric cancer and the effect of surgical treatment. Methods The clinical data of 94 cases with gastric cancer more than 70 years old were retrospectively analyzed. They were divided into two groups,50 cases in 70 to 75 years old group,44 cases in over 75 years old group. The concomitant diseases before operation, postoperative complications and mortality were compared between the two groups. Results The incidence rate of postoperative complication was 36.20%, and the periopera- rive mortality was 3.20%. The concomitant diseases before operation and postoperative complications in the over 75 years old group were higher than the 70 to 75 years old group （P = 0.03 ）, hut the postoperative mortality was not in- creased significantly（ P = 0.48 ）. Conclusion The risk of the surgical treatment is high in elderly patients with gas- tric cancer, and appropriate perioperative methods will help elderly patients to tolerate the surgical treatment.
