3D-OCT对45岁以下非典型中心性浆液性脉络膜视网膜病变的诊断价值

摘    要:目的 探讨三维光学相干断层扫描(3D-OCT)对45岁以上的黄斑区浆液性神经上皮脱离的眼底病变的诊断价值.方法 搜集双目间接检眼镜和眼底荧光血管造影(fundus fluoresecin angiogmphy,FFA)包含的浆液性神经上皮脱离的患者51例,应用TOPCON 3DOCT-1000+COLOR对其进行三维光学相干断层扫描(opitical coherence tomograph,OCT)检查,必要时结合吲哚菁绿血管造影(indocyanine green angiography,ICGA)进行分析.结果 51例浆液性神经上皮层脱离的OCT结果显示如下:(1)中心性浆液性脉络膜视网膜病变(CSC),27例(占52.9%);(2)渗出型老年黄斑变性(AMD),12例(占23.5%);(3)息肉样脉络膜血管病变(PCV),5例(占9.8%);(4)色素上皮失代偿(REPD),7例(占13.7%).脉络膜新生血管的定位和息肉样脉络膜血管病变的形态观察应结合ICGA.结论 3D-OCT快捷地从三维空间进行断层扫描,能发现45岁以上的黄斑区浆液性神经上皮脱离所包含的眼底病变,有利于对疾病诊断.

Abstract:Objective To analyze the value of 3D-OCT in diagnosis of macular serous neuroepithelial detachment in patients older than 45 years old.Methods TOPCON 3D-OCT-1000+COLOR were used to collect and analyze the results of macular serous neuroepithelial detachment in 51 patients older than 45 years old after fundus fluorescein angiography (FFA); and indocyanine green angiography (ICGA) if necessary.Results The results of FFA is as the follows: 1)central serous chorioretinopathy (CSC) was found in 27 patients (52.9%); 2)age-related macular degeneration (AMD) was found in 12 patients (23.5%); 3)polypoidal choroidal vasculopathy (PCV) was found in 5 patients (9.8%); 4)retinal pigment epithelial decompensation (REPD) was found in 7 patients (13.7%).Conclusions 3D-OCT is more accurate in diagnosis of macular serous neuroepithelial detachment in patients older than 45.
