分级脾蒂结扎法在儿童腹腔镜脾切除术中的应用

目的 探讨分级睥蒂结扎法在儿童腹腔镜脾切除术中的应用.方法 实施腹腔镜下分级脾蒂结扎法脾切除术8例,年龄5～14岁,平均年龄8.5岁;体重16～66kg,平均体重34.5 kg.结果 8例均在腹腔镜下顺利完成手术.手术时间90～150min,平均100min,术中出血10～200mL,平均50mL,住院时间5～7d,平均6.2d.8例均获随访,随访时间2～7个月,平均4.5个月.患儿术前症状完全消失,无并发症发生.结论 儿童腹腔镜下脾切除术中采用分级脾蒂结扎法可以减少术中出血,手术安全,效果好.

Application of fractional ligature of splenic stalk in laparoscopic splenectomy in Children

Objective To explore the application of fractional ligature of splenic stalk in laparoscopic spleneetomy in Children. Methods Eight children （Aged from 5 years to 14 years,mean 8.5years.Weighty from 16 to 66 Kg,mean 34.5Kg.）were performed with laparoscopic spleneetomy. Results All the patients were successfully performed under laparoseope.The duration of operation was 90-150 minutes （mean,100 minutes）, the intraoperation blood loss was 10-200ml （mean 50ml）,the time of hospitalization was 5-7 days （mean 6.2 days）.On follow-up of 8 cases for 2-7 months,all the symptoms of the children were disappearance without complication. Conclutions The technique of fractional ligature of splenic stalk in laparoscopic splenectomy in children can renduce the risk of blooding,and is safe and effective.
