微创冠状动脉旁路移植术中的备体外循环

目的 探讨非体外循环下冠状动脉旁路移植(OPCAB)术中的备体外循环。方法 2000年1月至2004年9月，603例计划实施OPCAB手术患者，体外循环采用“干备”和“湿备”2种方法。结果 603例患者中采用“湿备”23例，“干备”580例。术前临时改为体外循环下手术者“湿备”3例，“干备”19例，术中改为体外循环下手术“干备”1例。23例体外循环下手术患者无死亡。结论 “备体外”是OPCAB手术不可缺少的一部分，选择合适的“备体外”方法，有助于提高OPCAB手术的安全性。

Preparation for Extracorporeal Circulation in Off-pump Coronary Artery Bypass Grafting

OBJECTIVE To summarize the preparation for Extra corporeal Circulation in Off-pump Coronary Artery Bypass Grafting(OPCABG).METHODS 603 cases of preoperatively expected OPCAB were from Jan 2000 to Sep 2004. There were two types of preparation for ECC: 1. Wet-preparation: routine preparation for ECC with tubing system priming. 2. Dry-preparation: only connecting tubing system without priming.RESULTS In all of the 603 cases, there were 23 Wet-preparation, 580 Dry-preparation. 22 cases were converted to ECC pre-operation included 3 from Wet-preparation and 19 cases from Dry-preparation, 1 cases were converted to ECC from Dry-preparation during operation.CONCLUSION OPCABG can avoid the side-effects caused by ECC, it is the best choice of Coronary Artery Bypass Grafting, but this method still need ECC preparation during operations. In order to the safety, it was necessary to make proper preparation for ECC in expected OPCABG.
