快速康复外科在老年腹腔镜胆囊切除术患者围术期的应用效果

目的:分析快速康复外科应用于围术期老年腹腔镜胆囊切除术患者的临床效果。方法:选取本院行腹腔镜胆囊切除术的87例老年患者为研究对象,随机分为两组。对照组43例:围术期予常规治疗及干预措施;试验组44例:在围术期以快速康复外科理念作为指导进行临床干预治疗。对比两组患者治疗前后手术应激水平、血糖水平及术后恢复情况。结果:术后24 h试验组的Cor、WBC、IL-6及血糖水平显著低于对照组(P<0.05);试验组首次排气时间、下床时间及进食时间均显著短于对照组(P<0.05);试验组总并发症发生率显著低于对照组(P<0.05)。结论:快速康复外科可有效减轻老年腹腔镜胆囊切除术患者围术期的炎性应激及血糖水平,加快康复,减轻并发症的发生。

Application offast-track surgery in perioperative period of elderly patients undergoing laparoscopic cholecystectomy

Objective:To analyze the clinical effect of fast-track surgery(FTS)in elderly patients undergoing laparoscopic cholecystectomy during perioperative period.Methods:Eighty-seven elderly patients undergoing laparoscopic cholecystectomy in our hospital were randomly divided into two groups.43 cases in the control group were given routine treatment and intervention measures during perioperative period,while 44 cases in the experimental group were given intervention under the guidance of the FTS concept during perioperative period.The surgical stress level,blood glucose level and postoperative recovery before and after treatment were compared between the two groups.Results:The levels of Cor,WBC,IL-6 and blood glucose in the experimental group were lower than those in the control group 24 hours after operation(P<0.05);the time of first exhaust,time of getting out of bed and time of first eating in the experimental group were shorter than those in the control group(P<0.05);the incidence of total complications in the experimental group was lower than that in the control group(P<0.05).Conclusion:FTS can effectively reduce the perioperative inflammatory stress and blood glucose level in elderly patients undergoing laparoscopic cholecystectomy,accelerate the rehabilitation and reduce the incidence of complications.
